Objective:We determine the relative sensitivities and specificities of cytology andfluorescence in situ hybridization (FISH) for the detection of urothelial bladdercancer in order to evaluate application value of FISH in the screening of urothelialbladder cancer.Methods:From February2010-September2011, urine samples from60patients suspectedof having bladder cancer were collected before cystoscopy for immediate urinecytology and FISH analysis in the second affiliated hospital of Nanchang universityand the first affiliated hospital of Nanchang university. The criteria for FISHabnormality were determined by evaluating the urine specimens from20subjectswithout urogenital neoplasm.Results:(1). We statistically analyzed the proportion of cells with abnormal signal in thecontrol group and determined the thresholds. The amplification thresholds, the partialabsent and complete absent for P16gene detected by GLP P16DNA probes were5.8%,3.2%and3.9%respectively;The absent and amplification thresholds forchromosome17detected by CSP17DNA probes were3.8%and6.1%;4.1%and4.8%for chromosome3;4.2%and5.9%for chromosome7detected by CSP7DNArespectively.(2). The overall sensitivity of cytology and FISH was42.0%(19/45) and82.2%(37/45), the differences between FISH and cytology showed statistical significancein the sensitivity(P=0.000). Their specificity was93.3%(14/15) and86.7%(13/15)respectively, the differences between FISH and cytology showed no statisticallysignificant meaning in the specificity(P=1.000). The differences between FISH andcytology showed statistical significance in the sensitivity, low grade andnon-muscle-invasive cancer.(P<0.05).Conclusions: FISH technology has high sensitivity and specificity, it can improve markedlythe detection rate of urothelial bladder cancer,especially for the low stage and lowgrade tumors,and will be a new noninvasive and painless screening method forurothelial bladder cancer....
