Background and Objective:Progressive ischemic stroke (PIS) in shortstroke in progression(SIP),as a major subtypes of ischemic stroke, means after6h from the happening of actue cerebral infarction,whether or not acceptted thetreatment,but the patients’ conditon as to be gradually progressive deteriorationand even death,it’s tricky problems encountered in clinical work.Themechanism of SIP is complex,the disability rates and mortality rates of SIP arehigh.So it’s important to study on the risk fators of SIP, there is importantsignificance to take certain measures to prevent the occurrence of SIP.Thisreseach is intended for probing the risk factors of SIP by analysising SIP groupand non-SIP group cilincal data and serum hs-CRP level.Method:We selected96patients with acute cerebral infarction who were hospitalized in theneurology of first hospital of Jilin University from June of2010to Agust of2011. Clinical information including the history of gender, age, history ofhypertension, history of diabetes, random blood glucose, blood pressure,fasting and2h blood sugar after meals, triglycerides, cholesterol, high densitylipoprotein cholesterol, low density lipoprotein cholesterol, serum potassium,serum sodium and chlorine level, serum high-sensitive c-reactive protein levelof first day, third day, seventh day after being in the hospital, changes ofNHISS score. All metering data apply analysis of variance, count data apply χ2inspection, P<0.05statistically significant, P<0.01significant statistically.Results:In the group of96patients,45cases of SIP group,51cases of non-SIPgroup,incidence of46.8%.(1)We analysised the gender and age between SIPgroup and non-SIP group,there was no statistically significant.(2)We analysisedthe progress occurs in the SIP group,most of SIP occur within3d,after3dless.(3)Risk factors between SIP group and non-SIP group:There was statistically significant after statistical analysis between have or not havehypertention between SIP and non-SIP(P<0.01);we delaminated the history ofhypertension,there was statistically significant between history more than20-years and under20years(P<0.05), there was statistically significantbetween BP≥180/100mmHg and≤139/89mmHg、140/90≤BP≤159/99mmHg、160/100≤BP<180/100mmHg (P<0.05); there wassignificant statistically between diabetes and non-diabetes(P<0.01), afterdelaminting the history of diabetes,there was no statistically significant ofrandom blood glucose,fasting and2h after meals blood glucose;afterdelaminting the blood glucose, There was statistically significant betweenrandom blood glucose≥7.0mmol/L and3.9-6.1mmol/L(P<0.05), There wasstatistically significant between fasting blood glucose≥7.0mmol/L and3.9-6.1mmol/L(P<0.01)、and6.1-7.0mmol/L(P<0.05), There was statisticallysignificant between2h after meals blood glucose≥11.1mmol/L and6.1-7.8mmol/L and7.8-11.1mmol/L(P<0.05) There was statisticallysignificant of reducing of high density lipoprotein cholesterol and rising of lowdensity lipoprotein cholesterol(P<0.05).There was no statistically significantof serum potassium, serum sodium and chlorine level.(4) SIP group comparedwith patients with non-SIP group, there was statistically significant of serumhigh-sensitive c-reactive protein level of first day, third day, seventh day afterbeing in the hospital (P<0.05). Conclusion:(1)History of hypertension,historyof hypertension more than20years,BP≥180/100mmHg are risk factors ofSIP.(2)Diabetes,history of diabetes more than10years, random and bloodfasting glucose≥7.0mmol/L,2h after meals blood glucose≥11.1mmol/L arerisk factors of SIP,.(3)Rising of low density lipoprotein cholesterol andreducing of high density lipoprotein cholesterol are risk factors of SIP.(4)Elevated serum high-sensitivity c-reactive protein after acute cerebral infarction is a risk factor for SIP,the first day,third day levels are more significant.
