| ObjectiveTo observe the clinical efficacy of the three prescriptions (Bizheng â… , Bizheng â…¡ and Bizheng â…¢) created by my teacher in the treatment of arthralgia-syndrome, and to study their effects of analgesia and anti-inflammation. The study will provide clinical and pharmacological evidences for their clinical uses.Methods(1)Clinical studies:According to the principle of randomization,135cases of Arthralgia-syndrome patients comprises of RA, AS, KOA, LDH and cervical spondylosis were divided into treat group and control group, in which95and40cases were contained, respectively. According to the diagnosis of Traditional Chinese Medicine, the treat group was divided into three types (heat type, cold type and mixed cold and heat type), which were treated by three different prescriptions above, respectively.The Chinese patent medicine Wangbipian was used to treat for control group. When the Bizhengâ…¢ recipe was used, some different herbs were added according to symptom site, i.e. Pueraria and Clematis for neck and shoulder pain, Divaricate Saposhnikovia Root and Ramulus Cinnamoml for upper limb pain, Eucommia and Achyranthes for back and waist pain, Achyranthes and Menispermaceae for limb pain, Eupolyphaga and Centipede for severe pain.In addition to these Chinese herbs, other medicines were also used if necessary, e.g. Methotrexate was used for rheumatoid arthritis patients, Salicylazosul fapyridine was used for ankylosing arthritis, Leflunomide and Total Glucosides of Peony were given to the patients who difficult to control, Glucosamine Hydrochloride and Diacerein were used for knee osteoarthritis, Mecobalamin was used for limb numbness caused by cervical spondylosis or Lumbar Disc Herniation. Meloxican was given to the patient whose pain score was up to six. After2-4weeks of treatment, the total efficacy and improvement of symptoms and signs of two groups were compared and analyzed.(2)experimental studies:The hot-plate test of mice were used to observe the analgesic effects of the three prescriptions.The experiment of ear swelling induced by Xylene in mice were used to study the anti-inflammatory activities of the three prescriptions.Results(1) Clinical studies:In treat group, the total effective rate was96.84%and the significantly effective rate was40.00%. In comparison with the control group, the total effective rate was90.00%and the significantly effective rate was17.50%. The total effective rate of two groups were no significant difference, but the significant efficiency of treat group was higher than that of control group(P<0.05). After the treatment of the three prescriptions,the main symptoms were improved markedly,such as the number of joint pain,the degree of pain, the swollen degree of joint,the pressing pain, the function of motion,the skin temperature of joint,the daily life function, the degree of body cold or hot, the bi tter or dry mouth, the sleep, the pee and bowels conditions (P<0.05). The number of joint pain, the degree of pain, the pressing pain, the degree of body cold or hot, the bitter or dry mouth, the pee and the bowels conditions were also improved obviously after treatment of Wangbipian(P<0.05). The time of morning stiffness and the appetite were not improved in both treat group and control group. The improvement of the joint symptom, the whole body condition, and the total symptom in treat group of knee osteoarthritis were better than those of control group(P<0.05). The improvement of the whole body condition in treat group of Lumbar Disc Herniation was better than that of control group (P<0.05).There were no serious adverse events occurred after treatment.(2)animal studies:Compared with saline group, the low and high doses groups of the three prescriptiongs could improve pain threshold of mice in hot irritation(P<0.05), and the high dose had a better effect than the low one in each prescription. The effect of improving pain threshold of Bizheng â… group was better than that of aspirin group(P<0.05).In comparison with the saline group, the high and low doses of Bizheng I groups could significantly suppressed Xylene-induced ear edema in mice(P<0.01,<0.05), and its effect of high doses of Bizheng I group was even better than that of aspirin group(P<0.05).Conclusion(1) There are significant efficiency in treatment of arthralgia-syndrome by the three prescriptions,which are better than that of Wang-bipian.(2) All the three Chinese herb recipes have analgesic effect, and Bizheng I has anti-inflammatory effect which is better than that of aspirin.(3) The mechanism of the efficiency may be associated with pharmacological effect of three herbs prescriptions.(4) It has suggested that there are feasibility and effectiveness to identified all arthralgia-syndrome by a method distinguishing between cold and heat, And also confirmed that it is important to promoting blood circulation and removing blood stasis in treatmeng of arthralgia-syndrome. |
