Objective:By detecting the levels of serum BNP in patients with chronic heart failure, urinary BNP level, serum MMP-9levels, and determination of left ventricular end-diastolic volume (LVEDV),left ventricular end systolic volume (LVESV), left ventricular ejection fraction (LVEF) and other indicators with echocardiogram, analysis of its change and urine BNP and Relevance of serum BNP,further discussion correlation between urine BNP and ventricular remodeling, and guide diagnosis and treatment of heart failure.methods:According to inclusion and exclusion criteria in selecting research subjects and grouping, selected100cases of patients with chronic heart failure, according to the United States of New York Heart Association (NYNH).cardiac function grading classification:28cases in grade II.38cases in grade III, IV in34cases:simultaneous selection of30normal subjects as control group. All the subjects were supine rest for10minutes in the morning from cubital vein blood4ml. and at the same time the collection of urine4ml, all blood are collected on the anticoagulant tube,and then to3500r/min,10min centrifugal, separation of serum, serum and urine were all at-80℃conservation. Using enzyme-linked immunosorbent assay (ELISA) methods to detect heart failure group and the healthy control group. Serum BNP, BNP in urine and serum MMP-9level, echocardiogram diagnostic apparatus, determination of LVEDV, LVEF and other related parameters. Using SPSS16.0statistical software for all measured value of statistical analasis.Results:(1) single factor analysis of variance, chronic heart failure group than in the normal control group, serum BNP and urine BNP level elevated, and along with the deterioration of cardiac function in patients with heart failure and NYHA gradually increased, cardiac function class Ⅱ. class Ⅲ, class Ⅳ between normal controls and the differences were statistically significant(P<0.05); chronic heart failure group urine BNP and serum BNP and LVEF were negatively correlated(r=0.584,-0.591. P<0.05):(2) the level of BNP in urine and serum level of BNP were positively correlated (r=0.723. P<0.05):(3) single factor analysis of variance.compared with the normal control group, chronic heart failure group LVEDVI, RWT. LVEF had significant changes (F16.69,7.62.8.68, P<0.05). there were statistically significant (P<0.05); compared with the normal control group, chronic heart failure group the levels of serum BNP, serum MMP-9levels had significant change, there was statistically significant difference (F=14.46,40.46, P<0.05); serum BNP and serum level of MMP-9, LVEDVI were positively correlated(r=0.436,0.510, P<0.05); urine BNP and the levels of serum MMP-9, LVEDVI were positively correlated (r=0.462,0.452, P<0.05); urine BNP and serum BNP and RWT were negatively correlated (r=-0.227,-0.329, P <0.05).Conclusions:(1) along with the deterioration of cardiac function of chronic heart failure group, BNP level in serum and urine BNP level increased significantly;(2) the level of BNP in urine and serum level of BNP were positively correlated:(3) along with the deterioration of cardiac function, serum MMP-9and LVEDVI levels were significantly increased, RWT decreased, BNP in urine and serum BNP levels gradually increased, BNP in urine and serum level of MMP-9and LVEDVI showed a positive correlation, and RWT showed a negative correlation, urine BNP may and ventricular remodeling related.
