The Influence Of High Loading Dose Of Clopidogrel On The Levels Of SCD40L And Hs-CRP In The Serum Of The Patients With ACS After Interventional Procedures

Objective:Discuss the influence of High loading dose of clopidogrel on serum sCD40L、 hs-CRP levels in acute coronary syndrome patients after intervention operation.Method:We included216patients with acute coronary syndrome (ACS) and we used totally random method to divide the patients into two groups.We excluded26patients who combined with diseases that could influence serum sCD40L and hs-CRP levels.The number of the patients of the high loading dose of clopidogrel group was94.The number of the patients of the standard dose of clopidogrel group was96.The high loading dose of clopidogrel group uses600mg clopidogrel as loading dose and150mg clopidogrel daily for7days and75mg clopidogrel daily for maintenance treatment.The standard dose group uses300mg clopidogrel as loading dose and75mg clopidogrel daily for maintenance treatment. All patients have been detected serum sCD40L and hs-CRP levels before taking clopidogrel, at preoperative PCI, postoperative PCI for the first day, for the seventh day, for the thirtieth day.We haved observed the difference of the serum sCD40L and hs-CRP levels of the two groups.Results:l.The serum level of sCD40L of the high dose group was significantly lower than that of the standard dose group on the overall situation,and P=0.045, there is statistical significance.2.The serum level of hs-CRP of the high dose group was significantly lower than that of the standard dose group on the overall situation,and P=0.012.There is statistical significance.3.We haved detected the changing endency of the serum levels of sCD40L and hs-CRP.P<0.001.It has a significant correlation.Pearson correlative coefficient is0.128.The correlative coefficient is greater than0.There is a positive correlation.Conclusions:l.The serum level of sCD40L of the high dose group was significantly lower than that of the standard dose group at the overal situation.lt hints that the inhibition on the sCD40L of high dose of clopidogrel is stronger than that of the standard dose of clopidogrel.2.The serum level of hs-CRP of the high dose group was significantly lower than that of the standard dose group at the overal situation.lt hints that the inhibition on the hs-CRP of high dose of clopidogrel is stronger than that of the standard dose of clopidogrel.3.The changing trend of the levels of the serum sCD40L and hs-CRP is positive correlation,It hints when clopidogre linhibits the generation of the inflammatory reaction product of hs-CRP,it also inhibits the generation of the inflammatory reaction product of sCD40L.
