In recent years,the pace of the reform of primary health care system has been accelerated,and the rural doctors at the frontline,as a "gatekeeper" to maintain the health of residents,is playing an increasingly important role.Therefore,it is urgent and necessary to study the competency of the doctors in rural areas.In this study,through theoretical research and field investigation,to learn the current situation of primary clinicians' post competency in different regions of Jiangsu province,reveals the existing problems,in order to provide reference to further improve the ability of primary health services.Objective:Based on the investigation and evaluation of rural doctors' post competency in different medical institutions in Jiangsu Province,a detailed analysis of the local rural doctors' competency elements was collected.Revealing different levels of hospitals,different levels of rural doctors' job titles differences in the structure and competency so as to put forward countermeasures and suggestions to solve the plight of the rural doctors' practicing ability.Methods:Based on "The questionnaire of Chinese clinician competency",a questionnaire was designed to investigate the competency of rural doctors in Jiangsu province.The contents of the questionnaire include the basic information of the respondents and post competency.Basic information including the age,gender,title,education and working hours.The evaluating indexes of post competency include basic medical service ability,public health service ability,humanistic practice ability,professionalism and quality as well as studying and learning ability and so on.Results:(1)Rural doctors' competency indicators according to the importance of the order of basic medical service ability,public health service ability,professionalism and quality,humanistic practice ability and studying and learning ability.The respondents determine the degree of importance of the five factor of general practitioners' post competence are considered important and the average score ranks from 4.70 to 4.89 points.(2)There were significant differences in the 4 aspects of basic medical service ability,public health service ability,professionalism and quality,humanistic practice ability between surveyed objects from different levels of hospitals(P<0.05).There were significant differences in the 2 aspects of basic medical service ability and basic public health service ability between surveyed clinicians with different titles(P<0.05).At the same time,there is still a gap between the reality and the ideal state,especially the public health service ability and studying and learning ability are both the last two digits in the real situation.(3)In the township hospitals,the treatment of common diseases and chronic diseases,team cooperation,work experience in the secondary and tertiary hospitals,weekly self-study time and the ability of searching medical information is better than that of village clinics and the difference was statistically significant(?2=11.02,10.00,14.201,19.563,12.888,15.513,P<0.05);in the village clinics,referral service,home care,family health assessment,follow-up of three groups,the implementation of family planning mission,complaints and training opportunities is better than that of township hospitals and the difference was statistically significant(?2= 3.363,23.594,15.126,6.2,12.368,13.688,30.339,P<0.05).Conclusions:In recent years,the quality and the level of the rural doctors in Jiangsu area began to improve and optimize,but has not yet fully meet the job requirements.The development of the rural doctors' competency is not balanced and need to be improved and strengthened targetedly.According to the survey,basic medical service ability and public health service ability are among the top two in importance,but the studying and learning ability is the least important.Analysis to explore the "job competency" driven innovation of medical professionals' cultivation,selection and assessment.Administrative departments,medical institutions,medical colleges and other social organizations of various efforts,to jointly promote the rural doctors' competency,improve the basic medical and health service level.
