Objective:Using a cross-sectional survey methods to understand the epidemiologicalstatus quo of the hypertension of early renal damage in the community educationauthorities at the county level, analyse the risk factors associated with essentialhypertension of early renal damage, research the distribution of syndrome of thehypertension of early renal damage and its correlation with urinary microalbumin (MAU)to provide a scientific basis for the control strategies of hypertension of early renal damagefor county-level community in the northwest of Shandong Province.Methods: To investigate2395education institutions crowd (including serving andretired personnel) whose age of24to88years old in the Pingyuan county,DezhouCity,Shan-dong Province,throughing a standard form of questionnaire to collectage,gender,work unit,telephone,habits, family history,history of hypertension, personalhistory of chronic diseases, treatment status and permit designate grading quantitativeinformation. And according to standardized measurement method to measure the height,weight, waist circumference, hip circumference, blood pressure, central aortic pressure(cSBP), brachial-ankle pulse wave conduction velocity (baPWV), the main arteryelasticity (C1), small artery elasticity (C2), urinary microalbumin (MAU). Finallysummary data, using the non-conditional Logistic regression to analyse the risk factorsassociated with hypertension of early renal damage. At the same time, statisticsing TCMsyndrome distribution of hypertension of early renal damage, and comparing the differentsyndromes of urinary microalbumin (MAU) differences to explore the correlation betweenthe different TCM syndromes and the urinary microalbumin (MAU).Results:(1) Among2395education institutions crowd (including serving and retired) in the Pingyuan county,Dezhou City, Shandong Province,the hypertensive patientsare775cases, there are32.4%of the overall prevalence rate.The male and femaleprevalence rate respectively are34.6%and29.5%.Male prevalence was significantlyhigher than women (P <0.05);≤50years of age,51to60years,61to70years,≥71-year-old hypertension were20.0%,30.5%,40.4%,41.7%.With the age increasing,male and female prevalence rates of hypertension tends to increase (P <0.05).(2) Thereare71cases patients of hypertension of early renal damage among the775patients ofhypertension, the total prevalence rate is9.2%, in which male and female prevalence ratesare11.5%,5.5%, men is higher than women (P <0.05);The hypertension of early renaldamage prevalence rates among≤50years of age,51to60years,61to70years,≥71years of age respectively were1.0%,3.1%,8.4%,33.6%, With the age increasing, theprevalence rate of hypertension the of early renal damage increasing (P <0.05).(3)Logistic regression analysis showed:higher age (OR=1.082, P <0.05), smoking (OR=5.177, P <0.05), drug dependence difference (OR=0.641, P <0.05), the large pulsepressure (PP)(OR=1.114, P <0.05), the poor small artery elasticity (C2)(OR=0.291,P <0.05)are risk factors of hypertension of early renal damage.(4) There are mainly thehyperactivity of liver fire group,the accumulation of Phlegm and dampness group,the Yindeficieney with Yang hyPeraetivity group,the Yin-Yang defieiency group of the foursyndromes in hypertension of early renal damage, in which the hyperactivity of liver fire ishighest, the accumulation of Phlegm and damPness followed, the Yin deficieney withYang hyPeraetivity and the Yin-Yang defieiency at least, accounted for31.0%,26.8%,21.1%,21.1%; There are different levels of urinary microalbumin (MAU) amongdifferent TCM syndromes,in which the urinary microalbumin (MAU) of the hyperactivityof liver fire and the accumulation of Phlegm and dampness are higher, both no statisticallysignificant difference (P>0.05),but the Yin deficieney with Yang hyPeraetivity and theYin-Yang defieiency are lower,both no statistically significant difference (P>0.05),There are significant differences when pairwise comparisons (P <0.05).Conclusion:(1) The hypertension prevalence rate of the educational institutionscrowd is higher in Pingyuan County, Dezhou City, Shandong Province,that issignificantly higher than the national level,the prevalence rate of the hypertension ofearlyrenal damage is correspondingly increased. Prompt that hypertension andhypertension of early renal damage have become the region’s public health problem.(2)Logistic regression analysis: higher age, smoking, drug dependency,large pulse pressure (PP), the poor small artery elasticity (C2) are the risk factors in hypertension of earlyrenal damage.Suggesting that the county-level community patients with hypertension canbe prevented or reversed or delay the progress by giving up smoking, regular medication,lower pulse pressure, improving the small arterial elasticity.(3) There are mainly thehyperactivity of liver fire group,the accumulation of Phlegm and dampness group,the Yindeficieney with Yang hyperaetivity group,the Yin-Yang defieiency group of the foursyndromes in this country-level educational institutions crowd of hypertension of earlyrenal damage, in which the hyperactivity of liver fire is highest, the accumulation ofPhlegm and dampness followed, the Yin deficieney with Yang hyPeraetivity and theYin-Yang defieiency at least.What’s more, the different TCM syndromes and urinarymicroalbumin (MAU) may exist.And this have a certain guiding significance for ourclinical application of Traditional Chinese Medicine.
