Anylansis Of Clincal Of Drug-eluting Stent Thrombosis

Objective: To analysis clinical features and angiographic characteristics and exploreevaluate the efficacy of emergent PCI with the drug-eluting stent for stent thrombosis.Methods: Baseline and follow-up data of2365patients who underwent PCI, whichoccurred61cases of stent thrombosis, analysis clinical features, coronary angiography andstenting, chose219no stent thrombosis cases as controls in the same period. Results: Of2365patients,stent thrombosis was confirmed in61(incidence2.3%),49males and12females, average age (59.8±11.32) years old, including8acute thrombosis (incidence0.30%),23subacute thrombosis (incidence0.87%),15late thrombosis (incidence0.57%),15very late thrombosis (incidence0.57%), Predictive factors for stent thrombosis showedby multivariate analysis were smoking, old myocardial infarction, heart failure, narrowprocess of the fibrinogen level, bifurcation lesions, vascular lesions, stent overlap, stentdiameter. The patient have the risk factor of Men, heart failure, high levels of plateletfibrinogen, small diameter and long stents, the incidence of acute/subacute stentthrombosis (＜30days) more higher than late/very late stent thrombosis (＞30days).33cases of patients with typical manifestations of chest pain and ECG changes, wastreated by Interventional or Intravenous thrombolysis,56patients were cured survive,4cases of recurrent in-stent thrombosis, five patients died. Conclude: Stent thrombosis is acommon complication of coronary artery cavity of drug-eluting stent implantation. Thereason may be associated with multiple stents, small diameter and long stents placed, therisk factors (old age, diabetes, hypertension, smoking) failed to ideal control stent early,stop antiplatelet aggregation drug by oneself, High mortality and poor prognosis, involvedin the standardization of treatment and individual in the Interventional, Strengthen theanticoagulant is an important means of prevention of stent thrombosis.
