Objective:Determination of tumor necrosis factor-α(TNF-α), reactive oxygen species (ROS) level, epididymal sperm motility and morphology in epididymal fluid of patients with obstructive azoospermatism before and after levocamitine treatment, and compared these indexes with the control group, to further investigate the effects of levocarnitine on pregnancy outcome in patients with obstructive azoospermia treated with ICSI.Method:collect74cases of patients with obstructive azoospermatism treated with intracytoplasmic sperm injection(ICSI) at the second affiliated hospital of Shandong University of traditional Chinese medicine reproductive Center from February2010to October2011,randomly divided them into2groups:38cases were in levocarnitine group, taking levocarnitine before treated with intracytoplasmic sperm injection(ICSI) two times a day and1gram each time for three months;36cases are in control group, taking vitamine E before treated with ICSI two times a day and0.1gram each time for three months. Determination of tumor necrosis factor-α(TNF-α), reactive oxygen species (ROS) level, epididymal sperm motility and morphology in epididymal fluid of the two groups respectively before and after treatments, compare the rate of fertilization and adequate quality embryo, and pregnancy rates after PESA-ICSI between the two groupsadequate.Results:After levocarnitine treated, TNF-α, ROS levels, and epididymal sperm motility in epididymal fluid have a statistically significant difference compared with before(P<0.05). There was no significance in statistics in the control group between before and after the vitamine E treated (P>0.05)。After PESA-ICSI, the number of adequate quality embryo and the rate of adequate quality embryo of the levocarnitine group are significantly higher than that of the control group.Conclusion:Levocarnitine can down regulation TNF-α and ROS in the epididymal of patients with obstructive azoospermatism, accordingly improve the environment of the epididymis, and then elevate the quality of sperm and the rate of adequate quality embryo of ICSI.
