ObjectiveBy the contrast16cm z-direction and4cm z-direction coverage of acutecerebral infarction and vascular stenosis lesion detection rate and accuracy,toelevate the diagnostic value of16cm z-direction and4cm z-direction coveragespiral CT perfusion imaging in acute ischemic cerebral vascular disease.Methods75patients were recruited who underwent CT perfusion with320-row CTfrom Janaury to July in2011.The analysis of CT perfusion images were madeby two experienced radiologists. The number of narrow lesions of the brainperfusion abnormalities and intracranial vascular size branch in16cmz-direction coverage and16cm z-direction coverage are recorded andstatisticsed。The site of the lesions which are divided into groups:the perfusionabnormalities are grouped according to the lobes of the brain, intracranialvascular abnormalities according to the intracranial blood vessels branch out ofshape grouping.Results16cm Z-direction coverage can check out all the ischemic penumbra,217lesions be detected,4cm Z-direction coverage detectes135lesions;16cmZ-direction coverage to4cm Z-direction coverage greatly increased the number of disease detection,where the abnormal perfusion in frontal, parietal, occipital,cerebellar; and the peripheral vascular abnormalities (p>0.01). Which the82lesions not be found by4cm Z-direction coverage are all be found by16cmZ-direction coverage.5patients with Moyamoya disease were identified. Theradiation dosage of16cm Z-direction coverage is4.6,and less than5.6mSvwhen the4cm Z-direction coverage has the same image data.ConclusionsCompared to4cm Z-direction coverage,16cm Z-direction spiral CTscanner can provide diagnostic images of not only the location and range ofcerebral infarct penumbra, but also the abnormalities of peripheral vasculardisease, while significantly reduce the radiation dose, the dosage of contrastmedia and the time of examination.It also can diagnosis the distal vasculardisease, what apply to the diagnosis of non-infarct ischemic symptoms. Thefunction of whole brain data acquisition, can effectively advance diagnosis andreasonably preventive of the cerebral infarction.
