| Objective Objective: Evaluation of the effect of n-butylphthalide(NBP)on cerebral perfusion in patients with acute anterior circulation aortic cerebral infarction by using three variants of arterial spin labeling(3D-ASL).Methods METHODS:A total of 70 patients with acute anterior circulation cerebral infarction admitted and treated in Anshan Changda Hospital from January 2019 to January 2020 were screened, etiology diagnosis of artery atherosclerosis model,All patients met the 2018 Chinese guidelines for the diagnosis and treatment of acute ischemic stroke, etiology diagnosis according to TOAST(Trial of Org 10172 in Acute Stroke Treatment,TOAST)classification, onset time is greater than 4.5 hours less than 48 hours,The National Institute of Health Stroke Scale(NIHSS)ranges from 6 to 16.They were randomly divided into the treatment group(NBP injection 100 ml,intravenous point twice a day, changed to 2 NBP soft capsules, orally taken three times a day for 90 days after 14 days of treatment)and the control group(no NBP).Patients in both groups were given standard treatment including promoting blood circulation to remove blood stasis,anti-platelet aggregation, lowering lipids with statins and symptomatic treatment.Efficacy evaluation: NIHSS score,BI index and 90-day MRS score were compared between 2 groups before and 14 and 90 days after treatment to evaluate the improvement of neurological function. The Cerebral Blood Flow(CBF)and relative Cerebral Blood Flow(r CBF)were evaluated before and after treatment. The CBF and r CBF of 2 groups before and 90 days after treatment were compared by two statistical methods(c2 test and paired t test). Results NIHSS score Before treatment, there was no statistical difference in NIHSS score between the two groups(P>0.05),after 14 days and 90 days of treatment, the NIHSS scores of the control group and the treatment group showed a gradually decreasing trend, and there was a statistically significant difference between the two groups(P<0.05);BI index After 14 days and 90 days of treatment, there was a statistically significant difference in BI index between the control group and the treatment group(P<0.05),the MRS score of treatment group was significantly lower than that of control group after 90 days(P<0.05);On the 90 days after treatment, the affected CBF and r CBF in the treatment group showed a significant upward trend, and there was a statistically significant difference between the two groups(P<0.05).Conclusions Neurological deficits were significantly improved in patients with acute anterior circulation of the great artery atherosclerosis after 14 days and 90 days of NBP treatment, and cerebral insufficiency was improved in patients with acute anterior circulation of the great artery atherosclerosis after 90 days of NBP treatment,as evaluated by 3D-ASL. |
