| ObjectiveIn this study,modern medical imaging technology is combined with traditional Chinese medicine thinking,and patients with acute cerebral infarction(ACI)are scanned with 3.0T magnetic resonance three-dimensional arterial spin labeling(3DASL).To analyze the distribution law of TCM constitution of ACI patients and the correlation between 3D-ASL imaging technology and TCM constitution of ACI patients,to find out its hemodynamic law,and to distinguish and adjust the body,so as to provide theoretical basis for prevention and treatment of ACI from the perspective of TCM constitution theory,and provide reference for clinicians to carry out individualized treatment according to the specific conditions of patients.MethodA total of hospitalized patients who met the diagnostic criteria of acute cerebral infarction were selected from September 2018 to December 2020 in Teaching Hospitalof Chengdu University of TCM,and underwent MRI、MRA、DWI and 3DASL.and meet the diagnostic criteria of acute cerebral infarction.Collected general information,laboratory examination data and imaging data of patients;According to the standard of “TCM Constitution classification and determination table” issued by China Association of Chinese Medicine in 2009,the constitution of TCM in the included cases was classified and judged..and used SPSS 26.0 for statistical analysis.Results1.general data of ACI patients: in 112 ACI patients,the ratio of male to female is 1.8∶1,and there are more males than females.The age ranged from 24 to 89 years,with an average age of 63.29±12.04 years.Mainly middle-aged and old people;BMI is mostly distributed in normal and overweight;The main lesion site is the internal carotid artery system(anterior circulation),and the number of lesions is mainly multiple lesions.2.Distribution of TCM constitution types of ACI patients: single constitution is the main one;The main constitution types are mainly qi deficiency constitution,phlegm dampness constitution and Yin deficiency constitution.Among them,phlegmdampness constitution is the main factor in men and blood stasis constitution is the main factor in women.There is no significant difference in the distribution of the main constitution types in age,among which the middle-aged and the elderly are the most distributed,while the young are the least distributed.Patients with BMI≥24.0 were mainly of phlegm dampness constitution.There was no significant difference in the distribution of the main constitution types in the lesion location and the number of lesions.The lesion location was mainly the internal carotid artery system(anterior circulation),and the number of lesions was mainly multiple lesions(P>0.05).3.ASL perfusion image distribution of ACI patients: the ASL perfusion image of ACI patients is mainly hypoperfusion;There was no significant difference in the distribution of ASL perfusion images in gender and age,and all of them were mainly hypoperfusion(P>0.05).Patients without hypertension,hyperlipidemia and BMI≥24.0 were mainly hypoperfusion,patients with hypertension were mainly hypoperfusion and isoperfusion,and patients without hyperlipidemia and BMI<24.0were mainly isoperfusion.There was no significant difference in the distribution of diabetes mellitus,hyperhomocysteinemia,smoking and drinking among ACI patients on ASL perfusion images.4.Relationship between TCM constitution types of ACI patients and ASL perfusion images: The distribution of main constitution types of ACI patients in ASL perfusion images is statistically significant(P<0.05).Further comparison shows that the distribution of qi deficiency constitution and Yin deficiency constitution in ASL perfusion images is statistically significant,with hypoperfusion as the main factor,isoperfusion as the main factor;the distribution of qi deficiency constitution is negatively correlated with the level of ASL perfusion images(P<0.05).conclusion1.The ASL perfusion images of ACI patients showed hypoperfusion.2.The ASL perfusion images of ACI patients with hypertension were mostly hypoperfusion and isoperfusion.The ASL perfusion images of ACI patients with hyperlipidemia and BMI≥24.0 were mostly hypoperfusion.3.The ASL perfusion image of ACI patients with qi deficiency constitution is mainly hypoperfusion,and that of ACI patients with yin deficiency constitution is mainly isoperfusion.The distribution of qi deficiency constitution is negatively correlated with the level of ASL perfusion images. |
