BackgroundFatty liver disease (FLD) is one kind of reversible diseases and if it is treated earlier, liver function can be recovered. However, if it is not treated earlier, it might turn out to be irreversible liver disease such as liver cancer or liver cirrhosis. Existing studies showed that risk factors related to metabolism such as obesity, hyperlipidemia, type Ⅱ diabetes mellitus and hypertension were associated with FLD. In recent years, with the developing of living standard, the incidence rate of FLD takes on ascend trend. FLD has become China’s second big liver disease after the viral hepatitis and now it is threating our life greatly.Jinan railway administration is the backbone of the national railway network hub and coal transportation important channel. It is the basic guarantee for the national and Shandong province economic development and national defense construction.With the rapid development of the national railway, working rhythm is accelerated and working pressure is increasing in the employees of railway enterprise, especially for the workers on the first line. The prevalence rate of FLD and non-communicable disease(NCD) increases annually. More attention should be paid on the health status of the railway workers. It is important and necessary to understand the railway workers’ health status and formulate relevant measures for NCD prevention and control.However, the existing surveys of FLD were mainly cross-sectional epidemiological studies, and the trend analysis was seldom to be seen, and the sample size was much small. In order to further learn about the prevalence of FLD and relevant influencing factors, this study is thus designed to explore the prevalence trend of FLD and its affecting factors using the health examination data in railway employees in Jinan area from2006to2010.ObjectiveThe aim of the study is to analyze the prevalence of FLD in railway employees on the job in Jinan from2006to2010, to learn about the prevalence trend in the prevalence of FLD and relevant factors affecting in the prevalence rate of FLD, and thus provide basic information for prevention and control of FLD.The specific objectives of the study include:· To learn about the overall health and its trend of change in employees of railway enterprise in Jinan from2006to2010.· To examine the prevalence trend of FLD in employees of railway enterprise in Jinan in recent years.· To find out relevant factors that can affect the prevalence of FLD.· To put forward relative political suggestions on prevention and control of FLD and other major NCD on above results.Methods55938physical examination data of railway employees on the job in Jinan area from2006to2010were involved. The collected date include general characteristics, general examinations, internal medicine, surgery, blood biochemistry tests, abdominal Doppler ultrasonic, electrocardiogram,urine routine tests, liver function, renal function, blood sugar, and blood lipid.Methods of data analysis:Excel2000databases and SPSS16.0software were used to carry out the statistical analysis. Descriptive analysis was fist used. Trend analysis for the general health and the prevalence of FLD in employees of railway enterprise were secondly used. Single factor analysis and multivariate logistic regression analysis were used.Main Results1. The major health problems of Jinan railway system workers are overweight, high total cholesterol (TC), FLD, high triglycerides (TG), hypertension, obesity, high alanine aminotransferase (ALT), diabetes, whose incidence is higher than the average level of the Chinese people. The prevalence rate of hypertension, FLD, high TG, high TC, and high ALT shows out an increasing trend.2. The prevalence rates of FLD from2006to2010were28.10%,25.73%,28.76%,33.82%and39.04%respectively, and it was going up year after year.3.Results of univariate analysis showed out that related factors affecting the prevalence of FLD in railway employees on the job in Jinan including gender, age, unit, body mass index (BMI), penile brachial index (PBI), fasting plasma glucose (FPG), TG, TC and ALT.4.Results of logistic regression analysis indicated that male, greater age,unit, overweight, obesity, hypertension, diabetes, high TG, high TC, and high ALT, were all risk factors of FLD.Conclusion and suggestions1. The prevalence rates of major NCD among the railway employees in Jinan were high. There were many types of NCD and the prevalence rates of part of the NCD increased year by year. The health condition of the railway workers was not optimistic. Health intervention need to be further strengthened.2. The prevalence rate of FLD was going up year after year in Railway Employees on the job in Jinan. Factors influencing the prevalence rate of FLD were much extensive, and the prevention and treatment of FLD in railway workers should be strengthened.3.Many factors can affect the prevalence rate of FLD, obesity and overweight was one of the most important factor for FLD. The basic measures to prevent FLD include diet, moderate exercise, weight control, quitting cigarettes and alcohol.
