BACKGROUND Chronic urticaria is a common and frequently occurring clinical dermatology disease. The clinical character is repeatability. Although western medicine can relieve patients’pain quickly, but after stopping medication it will break out once again. The Traditional Chinese Medicine have the superiority which appears to be more safety and having a stable effect in long-term outcomes. But the premise is distinguish the pattern of syndrome exactly, otherwise the efficacy will not be obvious. Angelica yinzi is a common and traditional Chinese prescription in treating many kinds of clinical skin diseases which contains chronic urticaria as well. It was recorded in many medical formularies and medical notes of generations physicians. Among of them, relatively few have described about the indications clearly and definitely. Due to the influence of missing, confidential inheritance patterns of academic, schools of thought contending and so on, it is difficult to find a specific and unified adaptation symptom or Chinese medicine pathology.OBJECTIVE The aim of this study was to determine the efficacy, safety and recurrence rate of Angelica Yinzi in the treatment of chronic urticaria and to explore the key factors affecting the therapeutic effect.METHOD All patients in this study came from the patients who treated in Dermatology clinic of Guang An Men Hospital of China Academy of Chinese Medical Science from April2012to December2012.In accordance with the diagnostic criteria of "The guide diagnosis and treatment of chronic urticaria-2007version", we selected123cases of patients who met the inclusion criteria. After gathering the basic personal information, disease conditions, symptoms, information of four examinations, we prescribe the Angelica yinzi which is orally decoction, one dose a day, and continued for4weeks, observe the symptoms and lesions improvement. Using before and after self-control method, setting up the rating scale in order to determine the effect of integration and assess the safety.116patients of chronic urticaria were evaluated at ration and semi-ration on22clinical indexes by SSRI score. The date were analyzed with Logistic model. A rate of95%decrease or above in SSRI after therapy were "effective".RESULTS116subjects completed the study.32cases were cured,6cases were markedly effective,16cases were effective,62cases were ineffective. The total effective rate was46.4%. Among the54cases of effective patients, the total average score before treatment was (13.91±2.104) points while after treatment was (4.31±3.716) points. It appeared to have a significant difference by t-test(p<0.01). The itch degree’s score before treatment was (2.44±0.691) points while after treatment was (0.72±0.811) point. It appeared to have a significant difference by t-test(p<0.01). The frequency of attack’s score before treatment was (1.59±0.813) points while after treatment was (0.28±0.452) point. It appeared to have a significant difference by t-test(p<0.01). The amount of wheals’score before treatment was (2.09±0.784) points while after treatment was (0.67±0.673) point. It appeared to have a significant difference by t-test(p<0.01). The attacking time’s score before treatment was (2.04±0.726) points while after treatment was (0.61±0.811) point. It appeared to have a significant difference by t-test(p<0.01). The diameter of wheals’score before treatment was (2.54±0.719) points while after treatment was (0.80±0.810) point. It appeared to have a significant difference by t-test(p<0.01). The appearance of wheals’score before treatment was (2.52±0.606) points while after treatment was (0.69±0.843) point. It appeared to have a significant difference by t-test(p<0.01).The dermographism score before treatment was (0.70±0.983) point while after treatment was (0.56±0.839) point. There is no marked difference between the two group by t-test(p=0.1).The rate of adverse-event was6.7%; The recurrence rate was21.9%after stopping medication for3months. Multivariate analysis indicated that the disease induced by the change of environment or physical, chemical factors, diet, drugs, and blood deficiency score were associated with the effect of Angelica yinzi. Among all symptoms of blood deficiency, the multivariate analysis indicated that the tongue picture or pulse condition of blood deficiency, dry stool, complexion, lip or nail turning pale, dry skin were associated with the effect of Angelica yinzi.CONCLUSION1.There is no difference between effective group and ineffective group by treating with Angelica yinzi on sex, age, course of disease, having allergic history and family history or not.2.Angelica yinzi have a significant efficacy on improving itch degree, the frequency of attack, amount of wheals, attacking time, diameter of wheals and have no marked effect on the dermographism.3.Sorting by the degree of influence from large to small, it goes:the disease induced by the change of environment or physical, chemical factors, diet, drugs,the tongue picture or pulse condition of blood deficiency, dry stool, complexion, lip or nail turning pale, dry skin. If any of them exist, it appears to be more helpful by treating with Angelica yinzi.
