Clinical Significance Of CEA、CA125、CA19-9and AFP Serum Levels In Diagnosis Of Gastrointestinal Cancer

Objective:To investigate the clinical significance of elevated serum CEA、CA125、CA199and AFP levels in the diagnosis of gastrointestinal cancer.Methods:Retrospective analysis serum levels and positive rate of CEA, CA125,CA19-9and AFP in371patients with colorectal cancer,185patients with gastric cancerand210patients with chronic gastritis were diagnosed by pathology,which fromJanuary2012to December2012at the First Affiliated Hospital of Dalian MedicalUniversity.Results:1. The positive rate of CEA, CA125, CA19-9and AFP in patients withcolorectal cancer were43.9%,6.2%,21.8%and3.5%,respectively. The positive rate ofCEA, CA125and CA19-9in patients with colorectal cancer were higher than that ofpatients with chronic gastritis(P<0.05). The positive rate of AFP was no significantdifference in colorectal cancer and chronic gastritis(P>0.05).The positive rate of CEA,CA125, CA19-9and AFP in patients with gastric cancer were20%,5.9%,17.3%and7.6%,respectively. The positive rate of CEA, CA125, CA19-9and AFP in patients withcolorectal cancer were higher than that of patients with chronic gastritis(P<0.05).Thepositive rate of CEA in colorectal cancer higher than in gastric cancer,but the positiverate of AFP in gastric cancer higher than colorectal cancer(P<0.05). The positive rate ofCA125and CA19-9were no significant difference in colorectal cancer and gastriccancer(P<0.05).2. Each level section of the proportion of CEA in colorectal cancer higher than in gastriccancer(P <0.05). Each level section of the proportion of CA125and CA19-9were nosignificant difference in colorectal cancer and gastric cancer(P<0.05). The level sectionof the proportion of AFP rose more than4times in gastric cancer higher than in colorectal cancer, there was no significant difference in the rest of the same level(P>0.05).3. The positive rate of CEA in colorectal cancer of poorly differentiated group,moderately differentiated group and well-differentiated group compared with each otherwere significant(P<0.05). The positive rate of CA125, CA19-9in colorectal cancer ofpoorly differentiated group were higher than well-moderate differentiatedgroup(P<0.05).4. In colorectal cancer, the level section of the proportion of CEA rose more than4times in poorly differentiated group, moderately differentiated group andwell-differentiated group compared with each other were significant(P<0.05). The levelsection of the proportion of CA19-9rose more than4times in poorly differentiatedgroup higher than well-moderate differentiated group(P<0.05).5. In colorectal cancer, the positive rate of serum CEA in stage Dukes A, stage Dukes B,stage Dukes C and stage Dukes D was7.1%,43.3%,43%and75%respectively. Thepositive rate of serum CEA in stage Dukes D was higher than that in the otherstages(P<0.05). The positive rate of serum CA125in stage Dukes A, stage Dukes B,stage Dukes C and stage Dukes D was0,6%,3.3%and20%, respectively. The positiverate of serum CA125in stage Dukes D was higher than that in the other stages(P<0.05).The positive rate of serum CA19-9in stage Dukes A, stage Dukes B, stage Dukes C andstage Dukes D was3.6%,15.9%,25.6%and50%respectively. The positive rate ofserum CA19-9in stage Dukes D was higher than that in the other stages(P<0.05).6. In colorectal cancer, the level section of the proportion of serum CEA rose more than4times in stage Dukes D higher than that in stage Dukes A, stage Dukes B and stageDukes C(P<0.05). The level section of the proportion of serum CA19-9rose more than4times in stage Dukes D higher than that in stage Dukes A, stage Dukes B and stageDukes C (P<0.05).7. In gastric cancer, the positive rate of CEA in poorly differentiated group higher thanthat of well-moderate differentiated group(P<0.05). The positive rate of CA125,CA19-9and AFP compared in different degrees of differentiation was no significantdifference(P>0.05).8. In gastric cancer, each level section of the proportion of CEA, CA125, CA19-9andAFP in different degrees of differentiation were no significant difference(P>0.05).9. In gastric cancer, the positive rate of CEA, CA125and CA19-9in patients with lymph node and distant metastasis higher than that of patients without lymph node anddistant metastasis (P<0.05).10. In gastric cancer, the level section of the proportion of CEA, CA19-9rose more than4times in patients with lymph node and distant metastasis higher than that of patientswithout lymph node and distant metastasis (P<0.05).Conclusions:1.The CEA have a certain value of diagnosis in colorectal cancer,gastric cancer, but the positive rate is not high.2.Serum levels of CEA, CA19-9and CA125can be increased at different degree incolorectal cancer and gastric cancer, the low-level rises should also attach importance.3.Elevated serum levels of CEA and CA19-9may helpful to determing the stage andprognostic in colorectal cancer and gastric cancer.
