BackgroundPercutaneous coronary intervention (PCI) is one of the most commonly means of diagnosis and treatment for CHD which caused obvious psychological activation to the patients with CHD. In recent years, as the medical model transfers to bio-psycho-social medical model and the development of diplocardia medicine, the research with PCI and mental disorders is increasingly while the research in this area is still a blank in the small and medium-sized cities. So, compared to the increasing of patients with PCI, the study on anxiety and depression for the patients with PCI is very lag in our city.ObjectiveTo investigate the change of anxiety and depression before and after percutaneous coronary intervention in patients with coronary heart disease and the effect of psychological intervention for these patients.Methods1. In the study, we examined132patients in hospital, who underwent coronary angiography (CAG) to diagnose with CHD. According to the result of PCI, all of the subjects were divided into two groups:the stenting implantation group with65cases and non-stenting implantation group with67cases. We had the patients finished the SAS and SDS scales when they entered hospital and after percutaneous coronary intervention, and to explore the change of anxiety and depression in patients with angina pectoris of coronary heart disease before and after percutaneous coronary intervention. Then, the patients in stenting implantation group were random divided into two groups: psychotherapy group with33cases and control group with32cases. The patients in psychotherapy group were given mental intervention before leaving hospital. We had the patients finished the SAS and SDS scales when they have just had with perctaneous coronary intervention for four weeks, and to explore the effect of psychological intervention for the patients with anxiety and depression after percutaneous coronary intervention.Resultl.The comparison of baseline clinical characteristic such as age、sex、cultural degree、 Comorbidity、family history、economic condition and lipid level cases have no obvious statistical difference (P>0.05).2. The comparison of the two groups scores of anxiety and depression before PCI showed no statistically significant differences; then after PCI, the score of anxiety and depression in the stenting implantation group were significantly higher than that in non-stenting implantation group (46.4±6.7vs35.6±6.2)(42.7±7.1vs37.4±5.8). There were statistically significant (P<0.01)3.In the non-stenting implantation group, the anxiety score after PCI was reduced significantly than the score before PCI (35.6±6.2vs42.3±6.8),while the depression score after PCI showed no obvious changes than the score before PCI (37.4±5.8vs37.5±5.3).In the stenting implantation group, the anxiety and depression scores after PCI were significantly elevated than the score before PCI (46.4±6.7vs41.6±7.4)(42.7±7.1vs39.6±7.0)4.There was correlation between the score of anxiety and depression in the stenting implantation group(r=0.592,P<0.01).5.After mental intervention, the anxiety score in psychotherapy group was significantly reduced than in the control group (37.7±5.2vs43.3±8.3), there were statistically significant (P<0.01).While the depression score was decreased slightly than the control group (41.9±6.8vs44.4±9.9),there was no statistical difference between the two groups(P>0.05).6.In the psychotherapy group, the anxiety score changed significantly between before and after PCI (47.4±6.9vs37.7±5.2), while the depression score changed slightly(44.4±7.5vs41.9±6.8) between before and after psychological intervention.Conclusion1.The patients of stenting implantation are obvious symptoms of anxiety and depression than those of non-stenting implantation.2.There was correlation between the score of anxiety and depression for the patients with Percutaneous coronary intervention.3.Psychological intervention can effective reduce patient’s anxiety whom with Percutaneous coronary intervention, but there is no obvious effect on depression.
