Objective:To investigate the current situation of the utilization of maternal conservancy services and adverse pregnancy outcomes in Huaihua. To explore the effectiveness of health education intervention through mobile telephone message in pregnancy in order to elevate the quality of maternal conservancy services and reduce the incidence of adverse pregnancy outcomes and provide new information for making the model of maternal health education.Methods:①10counties were selected from Huaihua (total13counties) as the research region using stratified cluster random sampling method, the counties in each layers were randomly assigned to the intervention group and the control group (n=five counties). Objects of our study were maternal who registered in the Maternal and Child Health Hospital (stations) from January2012to October2012in these10counties, and she or her family members have mobile telephones.②KAP survey:50maternal were selected from each county to conduct KAP survey in the first prenatal registration and42days. Statistical analyses were using X2, t tests or variance analysis.Results:①Basic situation:From January2012to October2012,11477and12699were collected from the intervention group and the control group in the first prenatal registration, respectively. By the end of February2013,6603and6213maternal outcomes data were collected from the intervention group and the control group respectively, response rates were57.5%and48.9%. No significant differences were found in maternal age and gravidity (P>0.05); agricultural household and multipara in the intervention group were significant higher (P<0.001) than the control group.②Baseline survey were conducted to collect the utilization of maternal health services and pregnancy outcomes from June2011to December2011(before the intervention).9113maternal information were collected, including4971in the intervention group and4142in the control group. The results showed that pregnant women in the control group have a higher rate of detection compared with pregnant women in the intervention group in early pregnancy inspect, prenatal diagnosis, HIV antibody detection (P<0.05).③KAP survey:50maternal were selected from each county to conduct KAP survey (250in each group),205maternal were collected from the intervention group and227from the control group. The effective response rates were82.0%and90.8%, respectively. KAP survey results showed that:The differences in knowledge, attitude and behavior scores caused by intervention factor between the control and the intervention group were statistically significant (P<0.05), the rise of scores in the intervention group were higher than the control group (P<0.05).④Utilization of maternal health services:Results showed that the rate of prenatal screening, HIV antibody detection, prenatal care, prenatal care≥5times, folic acid supplement in the intervention group were higher than the pre-intervention levels.⑤Comparison of pregnancy outcomes:Before health education intervention, the intervention group (40.2%) and the control group (39.6%) have no significant difference in cesarean section rate; the cesarean section rate (35.6%) in the intervention group was significantly lower than the control group (40.1%) after health education intervention.Conclusion:①The utilization of maternal health services is low, the cesarean delivery rate was40.0%, the maternal mortality was38.75/105, the perinatal mortality rate was9‰.②Using SMS on maternal health education can promote maternal knowledge, attitude and behavior change; improve prenatal screening, HIV antibody testing, prenatal care, folic acid supplement; reduce the incidence of maternal cesarean section, promote the health of mothers and infants.③Using SMS to give health education for maternal is feasible.
