Study On Relationship Between HIF-1α In Serum And Cardiac Function Of STEMI Patients During PCI Treatment

Objective:Correlation of serum level of hypoxia-inducible factor-1α (HIF-1α)level with myocardial range, Cardiac troponin I (cTnI) level, B-type natriureticpeptide (NT-proBNP) level, left ventricular ejection fraction (LVEF) ofST-segment elevation myocardial infarction (STEMI) patient for percutaneouscoronary intervention (PCI)Methods:65cases of acute chest pain were enrolled in November2013-2014February The first hospital of Jilin University treatment in cardiovasculartreatment center.50patients with STEMI and acute PCI as acute myocardialinfarction group have symptom of acute chest pain whereas15patients, ascontrol group, found no obvious abnormalities in myocardial injury markerswith ECG and no narrowing in blood vessel during coronary angiogram test.Double antibody sandwich enzyme-linked immunoassay, chemiluminescenceimmunoassay to detect the blood of patients in both groups HIF-1α, cTnI,NT-proBNP levels, LVEF values obtained by MM methods, acute myocardial infarction patients through the admission ECG and calculate formulas Aldrichinfarct size; Establish a database. Data were analysed using SPSS19.0.Result:(1) For control group upon admission, HIF-1α level is72.45±23.49pg/ml;that of acute myocardial infarction group is433.26±238.17pg/ml; the value ofacute myocardial infarction group is higher than control group, P-value is ofstatistical significance (P<0.01).(2) For acute anterior myocardial infarction group upon admission, LnBNPis4.98±1.10pg/ml; that of acute inferior myocardial infarction group is4.93±1.27pg/ml; that of control group is3.97±0.70pg/ml; the differencebetween three groups is of no statistical significance (P>0.05); For acute anteriormyocardial infarction group, LnBNP24hours after operation is7.74±0.56pg/ml, which presents a difference compared to the value upon admission. Thedifference is of statistical significance (P<0.01); that of acute inferiormyocardial infarction group is7.10±0.52pg/ml. Compared to the value uponadmission, the difference is of statistical significance (P<0.01); for acuteanterior myocardial infarction group, there’s difference of LnBNP24hours afteroperation compared to acute inferior myocardial infarction group, which is ofstatistical significance (P<0.05). (3) LVEF for acute anterior myocardial infarction group is44±6.3%andthat of acute inferior myocardial infarction group is55.15±3.89%; LVEF ofcontrol group is69±1.58%; the difference between control group and acuteanterior myocardial infarction group, acute inferior myocardial infarction groupis of statistical significance (P<0.01); however, evident difference of LVEFbetween acute anterior myocardial infarction group and acute inferiormyocardial infarction group is of statistical significance (P<0.05).(4) Analysis the data with multiple linear regression analysis betweenHIF-1α peak value and cTnI peak value, LnBNP24hours after operation,myocardial infarction range,LVEF. F-test (P <0.01)：there was significantregression relationship. t test (P=0.005,0.024,0.004,0.307): the peak levels ofHIF-1α peak level, LnBNP24hours after operation CTnI peak level, myocardialinfarct size are positive correlation (P=0.005,0.024,0.004), the constant was-1208.501, regression coefficients were174.914,9.913,17.901. R2was0.606.Conclusion:(1) HIF-1α level upon admission of myocardial infarction group is higherthan control group.(2) For acute myocardial infarction group, there is no correlation betweenHIF-1α peak value and LVEF. (3)For acute myocardial infarction group, there is positive correlationbetween HIF-1α peak value and cTnI peak value, NT-proBNP24hours afteroperation, myocardial infarction range.
