Objective To investigate the relationship between TCM types of syndrome andclinical correlation factors of benign prostatic hyperplasia (BPH) accompanied withchronic prostatitis(CP) by TCM differentiation of symptoms and signs.Methods Differentiate 300 BPH cases to six TCM types of syndrome based on thestandard of the diagnosis and of TCM disease (National Standard). Record the clinicalcorrelation factors including age, course, urination irritation, IPSS (international prostatesymptom scoring), weight of prostate, Qmax, residual urine, PSA (prostate specificantigen). All the patients take the operation of TUPK (transurethral plasmakkinetic bipolarvaporization), the prostate tissue specimens are observed under light microscope. Theaccompanied inflammation seen in histomorphology is classified by prostatitishistomorphology classification standard. Statistics by SPSS11.0, analyze the relationshipbetween TCM types of syndrome and clinical correlation factors of BPH accompanied withCP.Results①In 300 cases, 208 cases are BPH, occupy 69.3%. 92 are BPHaccompanied with CP, occupy 30.7%. In BPH with CP, Dampness and Heat take thehighest place, Renal Deficiency and Blood Stasis next; these three types occupy 76.5%.②In BPH with CP, there is correlation between types of syndrome and age, course, urinationirritation and IPSS. Of age, there is significant difference between Dampness and Heat,Insufficiency of Splenogastric Qi and Deficiency of Renal Yang (P<0.01, P<0.05); thereis significant difference between Deficiency of Renal Yang and Deficiency of Renal Yin(P=0.011). Of course, there is significant difference between Dampness and Heat andDeficiency of Renal Yang, Blood Stasis (P=0.000, P=0.020). Urination irritation is severestin Dampness and Heat and Deficiency of Renal Yin. IPSS is higher in Renal Deficiencyand Blood Stasis.③Renal Deficiency and Blood Stasis may have relationship with clinicalindex.④Peri-gland inflammation takes the chief place in histomorphology. Theclassification of inflammation has relationship with types of syndrome. There are significant differences between Dampness and Heat and others types.Conclusion①The TCM pathogenesis is characterized with Dampness and Heat,Renal Deficiency and Blood Stasis. These three are the main types.②There is significantrelationship between Dampness and Heat and clinical symptoms. It is characterized withyounger age, shorter course and heavier urination irritation. The type of syndrome andclinical index may have some relationships.③There is significant relationship betweenDampness and Heat and inflammation in histomorphology. |
