Clinical Study Of Early Diabetic Nephropathy Qi Ge Sugar Granule Treatment

Diabetic nephropathy (DN) as a microvascular complications of diabetes mellitus (DM),which has the clinical manifestations of proteinuria,progressive kidney damage,and with the characteristics of insidious onset,progressive increase and poor prognosis,which seriously affecting the lives quality of patients with DN,and even has become the main cause of death in patients with DM.If they receive timely treatment of early DN,kidney damage can still be reversed.Currently, Western medicine mainly uses some methods like hypoglycemic,control blood pressure,regulate blood lipidand microcirculation to treat early DN,but there are still some limitations.In recent years,a large number of studies have shown that Chinese medicine in the treatment of early DN has a unique advantage.Based on summing up the experience of predecessors and integrating his years of clinical experience, instructor repute deficiency of qi and yin and stagnation of blood and toxin as the main pathogenesis of early DN,and thus develop the treatments of replenish qi and yin,promoting blood circulation and detoxication.According to treatments,he formulate the prescription which called Ge qi Jiang tang particle,and observe its clinical effect to early DN.Objetive:Through observe and compare the channgs in clinical symptoms, urinary albumin excretion rate (UAER), urinary β2-microglobulin (β2-MG),glucose,glycated hemoglobin Ale,lipids and other laboratory parameters before and after treatment with Ge qi jiang tang particle to cure the early DN,and evaluate its clinical effect. Safety of the particle is evaluated through observation of liver function test (ALT, AST),kidney function test(Cr, BUN),blood and urine routine test.Method:Sixty-four patients with early DN met the inclusion criteria are randomly divided into treatment group and control group,among the control group33cases,31cases treated.At first,all the patients have a period of two weeks of basis treatment(including the diet control, physical exercise and drugs selection,and then enter the experimental observation period).In the trial observation period,the control group is given basic treatment of diabetes and valsartan capsules,while the treatment group on the basis of the control group plus Ge qi jiang tang particle.After12weeks of clinical treatment,all the sixty-four patients with early DN are collected the TCM symptom score and urinary albumin excretion rate,urinary β2-microglobulin,glucose,glycated hemoglobin Alc,lipids,the function of the liver and kidney and the routine test of the blood and urine,and all the date are analyzed with statistic.Results:1. The comparison of the efficacy of TCM symptoms,the total effective rate of the treatment group was87.88%,but in the control group,the total effective rate was58.08%,there was a statistically significant between the two groups (P<0.05).The treatment group was better inreducing the TCM symptoms total score and improving the TCM symptoms like dry mouth,lassitude,numbness and tingling,heat in palms and soles,shortness of breath and lazy words,soreness of the waist and knees and frequent nocturia symptoms than the control group (P<0.05or P<0.01).2. The comparision of reduce the efficacy of UAER and urine β2-MG, the UAER and urine β2-MG in the two groups were significantly lower than before treatment,and compared to the control group,there were a significant differences (P<0.05or P<0.01).3. The comparison of FPG,2hPG, HbAlc for the two groups before and after the treatment, FPG,2hPG, HbAlc in the two groups after treatment than before treatment are significant improve (P<0.05),but there were no significant difference between the two groups (P>0.05).4. The comparison of the lipids situation for the two groups before and after the treatment,the two groups all can improve the TG、TC、 HDL-C and LDL-C significantly (P<0.05), and the difference was statistically significant (P<0.05),but the treatment group decreased the TG,LDL-C and elevated HDL-C were significantly better than the control group (P<0.05).5.During the test,to monitor the results of some subjects,which include the liver function (ALT, AST), renal function (Cr, BUN), and biood, urine routine were not find any adverse reactions. Conclusion:Ge qi jiang tang can effective improve the early DN patients’variety of clinical symptoms,and regulate blood sugar and improve dyslipidemia, also can reduce the UAER and urinary β2-MG significantly,and reduce the damage to glomerular and tubular damage,thereby protect the kidney’s function.delaying the progression of early DN,it aiso has the character of safe and reliable in clinical application.
