Background Esophageal cancer, as a usual and frequency encountered illness in China, espacially in northern Henan, is not only a great threat to psychological health status of patients, but also a terrible stressful event to the spouses of patients. Research shows that the psychological health of cancer patients and their relatives are interrelated,in the campaign against cancer, the spouses of cancer patients were believed to suffer together. With the development of the medical technology, the remission rate and survival rate of Esophageal cancer have a great improvement. But because of disease’s severity, long time treatment, over burden medical cost and the uncertainty of treatment results, it will bring a very serious negative psychological impact on patients and their spouses. Therefore, it is necessary to evaluate the psychological health status of spouses and patients with Esophageal cancer in northern Henan, and study the influential factors, and then provide the psychological intervention for the special groups to improve their quality of life,relize holistic care and entire family care.Objective To investigate the psychological health status of spouses and patients with Esophageal cancer in northern Henan, and study the influential factors of psychological health standard of them. To establish the psychological intervention model in order to provide reference for the multimodality therapy and clinical medical work of malignant tumors.Methods A total of120couples, patients who were hospitalized during the period from September2011to May2012, and their spouses, were selected from Oncology Department of the First Affiliated Hospital of Xinxiang Medical University, and randomly divided into two groups:intervention group(60)and control group(60).With the method of questionnaire survey,investigate their psychological health status by Symptom Cheeldist-90(SCL-90),and study the influential factors of psychological health standard with Socio-demographic questionnaire, Medical Coping Modes Questionnaire(MCMQ),Sample Coping Modes Questio-nnaire(SCSQ). Control group were given routine treatment and intervention group were given psychological intervention based on routine treatment, after4weeks of invervention, revaluated the psychological health standard of two groups and compared the difference.Results (1) The psychological health status of participants:the results of SCL-90indicates that patients’ total score(154.97±22.53), somatization(1.89±0.32), compulsion (1.98±0.48), depression1.75±0.44), anxiety(1.85±0.37), hostility(1.61±0.44) and horror (1.58±0.36) are all higher than normal model; the spouses’total score(149.16±20.25), depression(1.89±0.43) and anxiety(1.96±0.46) are all higher than normal model,while the score of paranoid (1.31±0.28) is lower than normal model(P<0.01).(2) The influential factors of psychological health standard:analysis of variance indicated that the psychological health standard of patientes was ralated to theri gender (F=29.047), age(F=17.325), degree of education (F=28.029) and monthly income (F=46.710), the mental health standard of spouse was related to theri gender(F=34.106) and degree of education (F=22.215)(P<0.01).Correlation analysis:patients’coping mode like "avoidance" has a negative correlation with anxiety and depression(P<0.01),"surrender" has a positive correlation with somatization, anxiety, depression and horror(P<0.05); Spouses’ positive coping mode has a negative correlation with anxiety,and compromising coping mode has a positive correlation with depression(P<0.01).Results Multiple linear regression model to analysis multiple factor of the psychological health status of patients,there were sevent variables entered regression equation,which includ the degree of education(t=-3.075,P=0.003), monthly income(t=-2.824,P=0.006),"avoidance"(t=-4.843,P=0.000) and "surrender"(t=2.905,P=0.004) coping mode of the patient, the spouse’ anxity(t=6.108,P=0.000), depression(t=4.239,P=0.000) and negative coping mode(t=2.102,P=0.038).(3) Effection of intervention:after treatment, the somatization, compulsion, depression, anxiety, hostility, horror and the total score of patients, the depression, anxity and the total score of spouses in the intervention group is better than before(P<0.05), while those in the control group had no change(P>0.05), the somatization,compulsion,depression, anxiety, hostility, horror and total score of patients,the depression, anxiety and the total score of spouses in the intervention group is better than that in the control group(P<0.05).Conclusions (1) The psychological health status of spouses and patients with Esophageal cancer in northern Henan was fare worse,varying degree and dimension of psychological health problem was widespread,which was related to many factors,should be taken seriously.(2) Psychological intervention is beneficial to improve psychological health status of spouses and patients with Esophageal.
