| Research purposesAnalysis the causes of elderly patients with femoral fracture between the rotor and injurymechanism, and compare the clinical effect of two kinds of operation method PFNA andartificial femoral fracture replacement to provide alternative clinical options for treatment ofelderly patients with femoral fracture between rotor.Research methodsFrom January2008to December2011,162elderly patients with femoral fractures betweenrotor were divided into observed group and control groups according to operation methods andages of patients. The observed group were treated by spin the proximal femoral intramedullarynail fixation (PFNA), while the control group were treated by the artificial femoral headreplacement. Surgical incision length, operation time, intraoperative blood loss, down load time,postoperative complications and Harris score an index analysis before and after3months,6months and a year are all in a detailed record.Results:Compared to artificial femoral head replacement group, the observed group’s incision length, time of operation, intraoperative blood loss, were lower than the control group(6.10±2.31vs18.32±3.63),(65.32±27.11vs118.56±38.62),(143.82±47.33vs473.54±86.73)(P<0.05); Thedown load time of the observed group were higher than the control group(6.33±0.74vs3.52±0.31)(P<0.05); All of the patients with no urinary tract infection, prosthetic loosening, coxavara, with no die, there was no significant difference on the postoperative complications for thetwo groups, such as pulmonary infection, urinary tract infection, deep venous thrombosis, bedsore,urinary tract infection, prosthetic loosening(3.70%vs4.94%),(0.00%vs0.00%),(1.23%vs1.23%),(0.00%vs1.23%),(0.00%vs0.00%),(0.00%vs0.00%),(0.00%vs0.00%)(P>0.05);The scores of Harris after3months,6months and a year after operation in the two groups werelower than before operation(70.32±5.10vs93.22±6.51),(82.76±6.82vs93.22±6.51),(84.81±8.33vs93.22±6.51),(73.62±6.33vs92.71±7.82),(81.36±9.21vs92.71±7.82),(84.62±8.61vs92.71±7.82)(P<0.05); The lowest scores of Harris in the two groups was in3months afteroperation; the Harris score before and after3months,6months and a year have not significantdifference for the two groups(70.32±5.10vs73.62±6.33),(82.76±6.82vs81.36±9.21),(84.81±8.33vs84.62±8.61)(P>0.05).Conclusion:The treatment of senile femoral fracture criteria between rotors is on the basis of stronginternal fixation of early ambulation, instability for senile femoral fractures between rotor, PFNAtreatment effect is better than artificial femoral head replacement. For the patients with severeosteoporosis, PFNA can still obtain satisfactory therapeutic effect. |
