ObjectiveThe choice of topics with myoma of uterus and the indication ofhysterectomy patients as the research object, through the comparison oflaparoscopic and abdominal hysterectomy effects on protein IgG,complement C3and C reactive protein immune ball and significance,laparoscopic and abdominal hysterectomy influence on the machine bodyimmunity.MethodsWe selected in Affiliated Hospital of Tai’an Medical College andTaishan District People’s Hospital in2012June-2013year in February60cases with myoma of uterus and the indication of hysterectomy patients,divided into2groups:30cases of patients with laparoscopic hysterectomy totreat (laparoscopic group),30cases of patients accepted open abdominalhysterectomy operation treatment (open group). Determination of serumimmunoglobulin IgG and complement C3and C reactive protein: a collectionof24hours before operation, after24hours and72hours every2ml venousblood, serum separation, Hitachi7020automatic biochemical analyzer wasused to immune transmission turbidimetry. At the same time, a detailedrecord of operation time, two groups of operation process of bleeding volume,postoperative exhaust time and postoperative highest temperature,postoperative hospital stay.Results1.Changes of immunoglobulin IgG in two groups before and afteroperation: the laparoscopic group before24hours,24hours after operation, postoperative number immunoglobulin IgG72hours (g/L) were10.915±1.2183,8.160±1.1506,8.653±1.1308. The open group before operation,24hours,24hours after operation, postoperative value of72hours of immuneglobulin IgG respectively (g/L):12.26±2.8246,6.8910±0.8589,7.5538±0.8879. Two groups of preoperative numerical immune globulin IgG were inthe normal range, the difference is not statistically significant (p>0.05). Twogroups of immunoglobulin IgG after24hours is24hours before operationwere significantly decreased (P <0.05), and laparotomy group than in thelaparoscopic group IgG decreased more obviously, there was significantdifference between the two groups (P <0.05).72hours and two groups ofimmunoglobulin IgG after operation compared with postoperative24hourshave increased, but still lower than the numerical24hours before operation(P <0.05), and the numerical control group than the laparoscopic group,there was significant difference between the two groups (P <0.05). Throughthe above data, can be found numerically the open group immunoglobulinIgG level decreased more significantly than that in the laparoscopic group.2.Changes of complement C3in two groups before and after operation:the laparoscopic group before24hours,24hours after operation,postoperative numerical complement C372hours respectively,(g/L):0.976±0.0517,0.854±0.0579,0.866±0.0523. The open group before operation,24hours,24hours after operation, postoperative numerical complement C372hours respectively,(g/L):1.009±0.078,0.776±0.0412,0.824±0.0303.Two groups of complement C3value of24hours were in the normal range,the difference is not statistically significant (P>0.05). Two groups ofcomplement C3after operation than before operation numerical24hours24hours were decreased significantly (P <0.05), and laparotomy group than inthe laparoscopic group decreased more significantly, there are significantdifferences between the two groups (P <0.05).72hours after operation intwo groups of complement C3value lower than24hours after the operationhas increased, but still lower than the value of24hours before operation (P <0.05), and laparotomy group than the low numerical laparoscopic group,there was significant difference between the two groups (P <0.05). Throughthe above data, can be found numerically the open group decreased C3levelis more significant than the laparoscopic group. 3. Changes of C reactive protein before and after operation in twogroups: laparoscopic group before24hours,24hours after operation,postoperative value of72hours of C reactive protein (mg/L) were:1.426±1.411222.1±8.061614.55±4.7984. The open group before operation,24hours,24hours after operation, postoperative value of72hours of C reactiveprotein (mg/L) were:1.4800±1.3119,36.1±9.3625,33.2±5.0067. Thelaparoscopic group and open group of preoperative value of24hours ofserum C reactive protein were in the normal range, no significant differencebetween the two groups (P>0.05). After24hours and two groups ofnumerical C reactive protein were significantly increased in24hours(P<0.05), and laparotomy group than in the laparoscopic group increasedobviously, there is significant difference between the two groups (P<0.05),after72hours and two groups of numerical C reactive protein were decreased,but also was significantly higher than that of numerical before operation(P<0.05), numerical control group C reactive protein is better than thelaparoscopic group, there was significant difference between the two groups(P<0.05). It is found, the laparoscopic group and laparotomy group after24hours and72hours, the value of serum C reactive protein were significantlyhigher than that before operation (P<0.01), and laparotomy group than theserum C in24hours and72hours after laparoscopic group are significantlyhigher C-reactive protein (P<0.01).4.Compared with the laparoscopic group and open group, the bleedingduring the operation, short operation time, less postoperative exhaust time,hospitalization time short, early postoperative highest temperature low.Conclusion1. Laparoscopic hysterectomy degree changes of immune globulin IgG,complement C3and C reactive protein were less than abdominal hysterectomy, through this research can be found,24hours of immunoglobulin IgG,complement C3value decreased after operation in two groups, but the opengroup than in laparoscopic group decreased significantly. Two groups ofpostoperative value of72hours of immunoglobulin IgG, complement C324hours after surgery than numerical increase, but still lower than the value of24hours before operation, and the open group numerical still thanlaparoscopic group. Numerical C reactive protein in laparoscopic and abdominal hysterectomy after24hours were significantly increased, and thenumerical open operation group was significantly higher than that inlaparoscopic operation group. Lower24hours after operation in two groupsof72hours of numerical C reactive protein than after surgery, but was stillhigher than that of numerical24hours before operation, and the open groupC reactive protein numerical still than laparoscopic group.2.Systemic reaction of laparoscopic hysterectomy than laparotomyhysterectomy operation is small, the injury of the body tissues, inhibitoryeffects on the immune function of small, is beneficial to the recovery ofinjured tissue and the tissues of the body than the open operation,laparoscopic operation advantage for the treatment of uterine fibroids havemore.
