Objectives:To explore the risk factors of postoperative venous thromboembolism of gynecological cancer patients.Methods:The data of 644 gynecological cancer patients with surgical treament were collected in the Taixing Hospital which is the artificial hospital of Bengbu Medical College from January 2009 to December 2014.Totally 32 cases of postoperative venous thromboembolism of gynecological cancer patients were defined as the VTE group,and the others were in the control group. The clinical data of the two groups were analyzed retrospectively.Result:(1)The age,body mass index in the VTE group were higher than those in the control group(all P<0.05).While the operation time,intraoperative blood loss in the two groups had no significant difference(all P < 0.05).(2)Modus operandi( c 2 =0.596,P=0.040)、Perioperative blood transfusion(c2 = 12.007,P=0.003)、Preoperative /postoperative chemotherapy(c2 =13.657,P=0.032)、Central Venous Catheters(c2 =5.056,P=0.025)、Department of internal medicine disease(c2 =12.007,P=0.011)、Tumor stage( c 2=5.294,P=0.021)increased the incidence of VTE obviously,adversely,postoperative anticoagulation( c 2 =8.522, P=0.004) decreased the incidence of VTE significantly.The other factors were not statistical differences between the two groups(P> 0.05).(3)Logistic regression analysis showed that the risk factors of postoperative venous thromboembolism of gynecological cancer patients were age over fifty years(OR=1.216,95%CI:1.012 ~ 2.435) 、 body mass index more than 25kg/m2(OR=1.873,95%CI:1.348 ~ 2.217) 、 open operation(OR=2.022, 95% CI: 1.719 ~3.223)、Preoperative/postoperative chemotherapy(OR=2.127, 95%CI: 1.474~3.743)、Department of internal medicine disease(OR=4.231,95%CI:1.550 ~ 11.546) 、Perioperative blood transfusion(OR=8.000,95%CI:2.054 ~ 31.159) 、 Tumor stage(OR=1.443,95%CI:1.002~2.671)、Central Venous Catheters(OR=1.954, 95%CI:1.275~3.275).Conclusion:VTE is one of the common complications of gynecologic cancer.The ages, body mass index,blood transfusion, complication with internal diseases,intervention chemotherapy,central venous catheter,open operation and the advanced stage of tumor were the the risk factors of postoperative venous thromboembolism of gynecological cancer patients,and the incidence of VTE can be decreased by anticoagulant therapy after surgery. Risk prediction models should be used for effective assessment of the risk factors for VTE before surgery, in order to make benefits of prophylaxis and treatment for high risk cancer patients.
