Objective:1.Analysis the differences in two-dimensional images and hemodynamic between patients with acute cerebral infarction of anterior circulation and the control group.To explore the correlation of carotid atherosclerosis and acute cerebral infarction.2. To evaluate the application of color Doppler ultrasound on the carotid atherosclerosis with acute cerebral infarction of anterior circulation. Methods:120 cases of neurology patients with CT or MRI proved acute anterior circulation infarction were choiced as acute anterior circulation infarction group,who were in the Second People’s Hospital of Anhui Province for treatment.At the same time,90 cases of non-infarction with CT or MRI excluded diagnosis were choiced as the control group. Sequioa512 color ultrasonic diagnostic instrument and 5 ~ 8 MHZ probe frequency were used in this study.The bilateral carotid artery diameter,inter median thickness(IMT),plaques lacation,plaques size and echo,carotid artery stenosis degree were observed by two-dimensional ultrasonography.The IMT thicken or equal to 1.0 mm is regarded as thickening,while it thicken or equal to 1.5 mm is regarded as plaque formation.To learn where the plaques were most found.Plaques are divided into low/echo plaques,mixed echo plaques and strong echo plaques according to different echo.The former two belong to unstable plaque,while the latter belongs to stable plaques.Carotid stenosis is divided into light,medium and heavy three levels according to the stenosis area.Peak systolic velocity(PSV),end diastolic velocity(EDV),early shrinkage acceleration time(AT),resistance index(RI),and pulsatility index(PI)of carotid artery were examed with color Doppler ultrasound.The measurement data expressed as(x±S)was tested by independent samples t-test using SPSS 17.0 statistical software for statistical analysis, while the count data was tested by chi-square.Statistically significant(P<0.05) was seen as difference. Results:1.The IMT in acute anterior circulation infarction group was thicker than in the control group(1.05±0.18 mm vs 0.79±0.15 mm,P<0.01),while the thickening of IMT detection rate was significantly higher than in the control group(79.2% vs 32.2%,P<0.001).The moderate-severe stenosis(19.2% vs 4.4%)and instable carotid artery plaque(43.3% vs 10%)in infarction group were obviously higher than those in control group(P<0.01).Arterial complete or incomplete occlusion were observed in severe cases.2.Most of the plaques were detected in the carotid artery bifurcation both in the acute anterior circulation infarction group and the control group(59.0% vs 52.0%).There was no statistically significant difference comparing the two groups(P>0.05).3.The blood flow parameters showed that the PSV and EDV of common carotid and internal carotid in acute cerebral infarction group of anterior circulation were lower than those in control group(P<0.05).RI and PI of internal carotid in acute cerebral infarction group of anterior circulation were higher than those in control group(P<0.05),while AT were lower than those in control group(P<0.05).There was no statistically significant difference between the numerical of RI,PI and AT in common carotid artery comparing two groups(P>0.05).Conclusion:1. Carotid artery is an important channel connecting the heart and anterior circulation.The inter median thickness of carotid artery,unstable carotid plaque,the degree of carotid stenosis is closely associated with acute anterior circulation infarction.2.The change of internal carotid artery blood flow dynamic was more sensitive than common carotid artery in acute anterior circulation infarction group.3.Color Doppler ultrasound can directly show the carotid artery morphological changes,and can provide abundant hemodynamic information.It is the first chioce for the carotid atherosclerosis.It has important significance to prevent before circulation infarction happening and assist etiology diagnosis.
