| Objective:1.To explore the clinical efficacy of "Hua Tan Xiao Yu" decoction in treating PLGC.2.Aimed at Survivin and Livin expression to explore the mechanism of "Hua Tan Xiao Yu" decoction in treating PLGC, in order to provide objective basis on cellular molecular level.Methods:1.261 outpatients diagnosed of CAG with IM were selected and were given "Hua Tan Xiao Yu" decoction treatment, twice a day for 6 months. Electronic gastroscopy and pathological histological detection were performed before and after the treatment.2.96 male SD rats of 6-8 week old were randomly divided into normal group and model group. N-methyl-N-nitro-N-nitrosoguanidine, ranitidine and irregular diet were used for preparing PLGC animal models. After a successful modeling, the rats were randomly divided into model group, low-dose group, middle-dose group and high-dose group of Chinese medicine, and western medicine group.28 week later, the gastric mucosa of gastric body and antrum were picked and stored. HE staining was used to identify pathological changes of gastric mucosa, meanwhile, using immunohistochemical methods contrast the expression of Survivin and Livin apoptosis inhibitor protein in gastric mucosa of rats in each group.Results:1.After this therapy,104 patients recovered,129 patients got better and 28 patients had no efficacy, the total efficiency was 89.27%. There were 218 cases of chronic atrophic gastritis(CAG) got better, the improve rate was 83.52%. Compared to the pathological histological result of themselves before this treatment, the result has statistically significant (P<0.01). In addition,177 cases of intestinal metaplasia(IM) was improved, and the rate was 67.82%. Compared to the pathological histological result of themselves before this treatment, the result has statistically significant (P<0.01). The therapeutic effects of CAG and IM were statistically different (P<0.01).2.HE staining results suggested that no PLGC changes in normal group, while all model group appears PLGC changes. PLGC pathology got better in middle-dose and high-dose "Hua Tan Xiao Yu" decoction groups (P<0.05), especially the high-dose group was even better than other therapy groups (P<0.05).PLGC pathology therapy of Folie Acid Tablets group was better when compared with the model group (P<0.05). In addition, immunohistochemistry showed that the expression of Survivin and Livin protein in model group were significantly higher than the normal group (P<0.01). Comparing each "Hua Tan Xiao Yu" decoction group with the model group, the Survivin and Livin expression decreased in a dose-dependent manner (P<0.01). The suppression effect of high-dose group was significantly stronger than other groups (P<0.01).Conclusion:1."Hua Tan Xiao Yu" decoction has obvious efficiency in treating PLGC. Besides, it was more effective on gastric mucosal atrophy than IM.2."Hua Tan Xiao Yu" decoction was better than Folie Acid Tablets in treating PLGC, and its mechanism may closely related to the inhibition of Survivin and Livin apoptosis inhibitory protein expression. |
