| ã€Objective】Through building a model of the rat which has both obstructive jaundice(OJ) and biliary tract infection(BTI), this experiment aims to address blow two issues: 1) use Procalcitonin(PCT) to evaluate if the patient with biliary tract obstruction has latent Biliary Tract Infection(BTI). which also provides a theoretical basis for the prognosis that early latent BTI is caused by clinical BTI treatment; 2) to provide clinical value for preventing and curing early latent BTI and its side effects i.e. systemic inflammatory response syndrome(SIRS). multiple organ dysfunction syndrome(MODS), sepsis, and etc.ã€Methods】Sanitary outbred male SD rats. weighing from 300-350g(the rats are provided by Kunming Chushang Technology Co.,Ltd. The rats are divided into 3 groups(each has 40 rats) at random: A is regular group; B is sham-operation group. in which SD rats only receive abdominal closure; C is biliary tract obstruction group. in which the Common bile duct(CBD) is located and given incomplete ligation with No.0 string around the joint of left and right hepatic ducts(beneath 0.5cm). In Group C. the test takes vein blood sampling at certain time(1d, 3d, 7d, 14d). takes liver sample. draws bile for culture, eliminate bile to culture negative rat. and executes 10 rats at certain time(1d〠3dã€7dã€14d). All test rats receive vein blood sampling and HE staining in liver sample.ã€Results】(1).Compared with Group B, the WBC〠PCT〠ALT. and TBIL in Group A. have no significant change.which has no statistical difference(P ï¹¥0. 05).(2).ALT Test: The ALT in Group C is much higher than Group A and Group B. which has statistical difference(P <0. 05).(3).TBIL Test: TBIL in Group C is much higher that Group A and Group B. which has statistical difference(P <0. 05).(4).WBC Test: in early stage(1d,3d), WBC in Group A. Group B and Group C have no obvious difference. which has no statistical difference(P ï¹¥0. 05); in later stage(7dã€14d). WBC in Group C is much higher than Group A and Group B.which has statistical difference(P <0. 05).(5).PCT Test: at time of 1d, PCT in Group A. Group B and Group C have no significant change, which has no statistical difference(P ï¹¥0. 05); at time of 3dã€7d and 14 d.PCT in Group C is much higher than Group A and Group B. which has statistical difference(P<0. 05).(6).Liver Sample’s Pathological Changes: SD rat livers in Group A and Group B remain stable in each time without any particular changes. In Group C. rat liver has no obvious infection symptom at time of 14 d. and the major feature is obstructive jaundice’s pathological change: rat liver has no change in the first day; rat liver has no change in the third day; rat liver begins to change in the seventh day–liver cells have scattered denaturation and necrosis at first. along with infiltration, edema. clear cytoplasm; then they have net-like and feather-like denaturation. along with bile vessel epicyte hyperplasia near the portal areas, but there is no obvious cholestasis. As obstruction takes longer. bile vessel epicyte hyperplasia increases. more liver cells have denaturation. the central vein area have vast denaturation. along with obvious net-like and feather-like denaturation. the centrilobular portion begins cholestasis. hyperplasia gradually expands and spreads into centrilobular portion. and part of the liver is dead at the later obstruction stage.ã€Conclusion】As the WBC in biliary tract obstruction group has no obvious change and the PCT specificity becomes higher. the PCT can be used as an accurate indicator to evaluate if the patient with biliary tract obstruction has early latent BTI. and it also can be used to diagnose early biliary tract infection. |
