| Objective:To investigate the dynamic hip combined with bone cement and artificial femoral head replacement the clinical curative effect of the treatment of senile fracture with serious osteoporosis unstable intertrochanteric. Methods:A retrospective analysis of patients with January 2010 to December 2012 December 40 cases over 75 years old of unstable intertrochanteric fracture with dynamic hip joint, Divided into dynamic hip screw group and artificial femoral head replacement group,20 cases in each group, two groups of patients with the intraoperative amount of bleeding, intraoperative and postoperative blood transfusion, hospitalization time, time in bed and complications were compared, and according to the function of hip joint, the dynamic hip screw group and artificial femoral head replacement group the Harris score were compared. Results: All the patients were followed up, the follow-up time ranged from 12 to 48 months, average 22 months. Two groups of patients with operation time and intraoperative bleeding and postoperative blood transfusion and hospitalization time no significant difference (P> 0.05).Artificial femoral head replacement group the postoperative recovery time in bed than dynamic hip screw groupies reduced, the difference was statistically significant (P< 0.05). Dynamic hip screw group 2 cases complicated with coxa vara, the excellent and good rate was 90%. Artificial femoral head replacement group with 1 cases of coxa vara, the excellent and good rate was 95%. Dynamic hip screw group (91.8+6.1), artificial femoral head replacement group (92.5+5.6). No statistically significant differences in the Harris scores of the two groups (P> 0.05). Conclusion: Dynamic hip screw internal fixation operation and artificial femoral head replacement operation for elderly patients with unstable intertrochanteric fracture treatment have its advantages, as long as master the indications, can achieve good results. |
