Background:Osteoporosis is a disease that weakens the bones to the point that they become break easily. It has high morbidity all over the world. With the population aging, osteoporosis increased the economic burden on society, which recogized as one of the serious public health problem. China is not only the country with the larget population in the world, but also the only one whose aging population is over one hundred million. And the number of our patients with osteoporosis is very high. In the meantime, it’s often accompanied with knee osteoarthritis, with the same stage of age. The destruction of the bones is the reason of prosthesis loosening while the strength of the surface of bones is reduced gradually, the late complications after totle knee arthroplasty, it’s the common reason of knee revision after operation. So how to do the totle knee arthroplasty for the knee osteoarthritis patients with osteoporosis, and the preoperative treatment is the important research subjects of joint surgery recently. Methods:We retrospectively compared the data of22patients who were diagnosed as osteoporosis in the Second Affiliated Hospital of Zhejiang University. We treated them by total knee arthroplasty. The bone mineral density of all these patients was measured by dual energy X-ray absorptiometry. All the patients were treated by legacyposterrior stabilization Flex knee arthroplasty. Anti-osteoporosis drugs, like alfacalcidol soft capsulesi and calcium tablets, were used two to three months later of the operation. All patients were followed up with knee joint functional score (the hospital for special surgery, HSS score). Comparaed the clinical characteristics and scores between before and after surgery, we analyse the characteristics of totle knee arthroplasty with osteoporosis, explore the perioperative treatment.Results:The results of early term follow-up showed that cuts of all the patients were A healing, and all the fixation were stable with no complications. Reexamined at3months, without prosthesis loosening. The function of the knee joint was measured by HSS score. The score of pre-operation was40.4±1.1, and post-operation was raised to95.4±0.4. There was a significant difference (P<0.05).Conclusion:To the patients with osteoporosis, total knee arthroplasty, can improve their knee function and ease the pain. While the strength of the surface of bones is reduced gradually, the individual prosthesis、careful operation and specific rehabilitation should be noticed. Longer follow-up, prospective study with a large number of samples, comparison with contrast group were needed to dismiss the reduced rate of activited because of aging, which contributed to the further research of the relationship betweem osteoporosis and total knee arthroplasty.
