| Objective To study the effectiveness of family partnership intervention care in patients with hypertension and type2diabetes in community, including family function, knowledge and attitude and practice(KAP) toward healthy lifestyle, relational physiological and biochemical outcomes, and quality of life(QOL).Methods Seventy two patients with hypertension and type2diabetes in XueTian community of Nantong who met the study criteria and volunteered to participate in the study were included. The patients were randomly assigned to the FPIC group (n=36) and the conventional group (n=36) using random number table.In the FPIC group, the patients and their family members both participated in the study at the same time, while in the conventional group, only patients took part in the study. The forms of intervention included brief individual educational sessions, group educational sessions, normal intervention in community, telephone calls and text messages intervention. The contents of intervention were basic knowledge of the disease, medication, physical activity and exercise, diet, self-measurement. Brief individual educational sessions were carried out every month till the6th month, group educational sessions were held at the1st month and the6th month, normal intervention in community were conducted each month. We used hypertension and diabetes handouts and Power point of health education as teaching aids. The whole period of this study was delivered over12months,6-month intervention period and6-month follow-up period. Before intervention, the6th month and the12th month collected the outcomes of family function, the KAP toward healthy lifestyle, relational physiological and biochemical outcomes, and QOL.Results67of72patients finished the study, of which34were in the FPIC group and33in the conventional group. There was no demographic difference between the two groups (P>0.05).The scores of family function in the FPIC group were higher than the conventional group at the6th month and the12th month, and the difference were significant(P<0.05). In the FPIC group, patients’family function were improved at the6th month and the12th month compared with the baseline(P<0.05).The KAP toward healthy lifestyle of the FPIC group were better than that in the conventional group at the12th month(P<0.05). In the FPIC group, The KAP toward healthy lifestyle were improved at the6th month and the12th month compared with the baseline(P<0.05). In the conventional group, the KAP toward healthy lifestyle were improved at the6th month compared with the baseline(P<0.05), the KAP toward healthy lifestyle were decreased at the12th month compared with the6th month (P<0.05).Systolic blood pressure(SBP), diastolic blood pressure(DBP) and blood glucose levels in the FPIC group were lower than that in the conventional group at12th month(P<0.05). In the FPIC group, compared with the baseline, SBP and DBP levels were improved at the6th month (P<0.05), SBP, DBP and blood glucose levels were improved at the12th month (P<0.05). In the conventional group, SBP and DBP levels were improved at the6th month compared with the baseline(P<0.05), SBP and DBP levels were increased at the12th month compared with the6th month (P<0.05).Social functioning(SF), bodily pain(BP), role physical(RP) and role emotional(RE) in the FPIC group were improved than the conventional group at the12th month(P<0.05). In the FPIC group, compared with the baseline, PF, SF, RP and general health(GH) were improved at the6th month (P<0.05), Physical functioning(PF), SF, BP, RP, RE and GH were improved at the12th month (P<0.05), BP and RE were improved at the12th month compared with the6th month(P<0.05). In the conventional group, PF, SF and GH were improved at the6th month compared with the baseline(P<0.05), SF and GH were decreased at the12th month compared with the6th month (P<0.05).Conclusions FPIC can increase family function in patients who were with hypertension and type2diabetes, while improve the KAP toward healthy lifestyle, some relational physiological and biochemical indicators and patients’QOL, but patients also need some guidance from nurses timely. Family members can play an important role in community health management team for patients with hypertension or diabetes. |
