Objective:To investigate and evaluate the Long-term effects about grade II and grade IV hemorrhoids between PPH and Milligan-Morgan method, though a retrospective and controlled clinical research.Methods:To collect the hemorrhoid cases from Chengdu Special Hospital of Anus and Large Intestine during2007.1to2012.12, which had been treated with PPH or Milligan-Morgan method, and all cases should match the diagnostic standards of Ⅱ~Ⅳ degree hemorrhoids. To follow-up the current status of the patients by telephone. Fill in the questionnaire and arrangement the data. To analysis and process the digital record with SPSS18.0.Results:In this Observation, there are1015valid questionnaire, in which509cases have been operated on by PPH while506cases by Milligan-Morgan method. Through the retrospective analysis of the data, we finally have the following results.1ã€The long-term complication:On aspects of mass prolapse, anal incontinence, endless defecate feeling, the incidence and index score in PPH group is significantly lower than that in MM group(p<0.05).On aspects of fecal urgency and thin stool, the incidence of MM group is significantly lower than the PPH group (p<0.05), index score of fecal urgency, thin stool and anal discomfort is lower than the PPH group (p<0.05). On aspects of anal pain, anal bulge, hematochezia anal secretions, anal itching, constipation and diarrhea, the two groups have no difference neither in the incidence nor index score(p>0.05).2ã€Signs and Symptoms: On aspects of anal epicanthus sizeã€internal hemorrhoids condition〠anal smaller, the incidence and index score in PPH group is lower than MM group(p<0.05). On aspects of Anal epicanthus swelling and perianal erosion, the two groups have no difference (p>0.05).3ã€Total complication and symptoms rate:the PPH group is lower than the MM group on total symptoms rate(p<0.05). On total complication rate, the two groups have no difference (p>0.05).4ã€Postoperativ recurrence and Reoperation rate:the PPH group is lower than the MM group(p<0.05).5ã€Patients satisfaction:the PPH group is much higher than the MM group(p<0.05).Conclusion:With fewer signs and symptomsã€lower postoperative recurrence rate and reoperation rate, less anal dysfunction and higher patients satisfaction, the operating method of PPH has better long-term treatment than traditional operating method. |
