Serum Irisin Level And Its Correlation With Insulin Resistance In Patients With Type2Diabetes Mellitus

Background:The incidence of type2diabetes mellitus has been growing rapidly, and researches on diabetes related metabolic diseases are also increasingly deeper. The important role that adipose tissues and muscles played in metabolism received much greater attention. Cytokines secreted by adipose tissues and muscles are called "adipokines" and "myokines", which can keep the balance of our body. Some even postpone or accelerate the development of insulin resistance. In2012, Spiegelman newly defined a cytokine called irisin,(named after Iris), which very likely to contribute to the prevention of obesity and diabetes occurrence.Objective:To investigate the association between serum irisin and insulin resistance.Method:82subjects with type2diabetes mellitus (T2DM) were selected from patients in Department of Endocrinology, the First Affiliated Hospital, Zhejiang University School of Medicine (including48men and34women), and62control subjects (NC) were selected from healthy people in medical centre (including33men and29women). Anthropometric indexes, metabolic parameters and serum irisin levels were measured. The data were analyzed using student-t test, Pearson correlation analysis and multiple linear regression analysis.Result:Serum irisin levels were significantly decreased in T2DM group (1.52±0.64) ng/L compared with NC group (2.65±0.64) ng/L (t=-10.233, P<0.05). Pearson correlation analysis showed serum irisin levels of T2DM were inversely associated with waist hip ratio (WHR), postprandial blood sugar and homeostasis model assessment of insulin resistance (HOMA-IR)(r=-0.603、-0.295、-0.22, P<0.05). Other clinical parameters, including disease duration, gender, age, smoking habit, diabetic complications, other complications, body mass index (BMI), blood pressure, fasting blood glucose (FBG), glycated hemoglobin Ale (HbAlc), fasting insulin (FINS), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), serum total cholesterol (TC), total triglycerides (TG) and atherosclerosis index (AI), were not correlated with irisin levels (r=-0.009、-0.035、0.045、0.014、0.118、-0.005、-0.029、0.112、0.095、-0.163、-0.088、-0.107、-0.088、-0.121、-0.008、0.131、0.123, P>0.05). Multiple linear regression analysis showed that after adjusting for gender, age, blood pressure and complications, WHR was significantly negatively associated with serum irisin levels, and WHR was the negative independent determinant of serum insin.Conclusion:In this study, we found that serum irisin levels were decreased in patients with type2diabetes mellitus and associated with the development of insulin resistance.
