Objective:To explore the relationship of serum APN level and CAC in clinical patients by case-control study, and the correlation with its risk factors. the study may provid value for clinical CAC prediction, diagnosis and treatment.Methods:Altogether 103 patients was included from our department between October 2013 and May 2014. No calcification, mild, moderate, severe calcification group were divided according to coronary angiography results and the severity of calcification classification.The serum total cholesterol(TC), triglycerides(TG), high-density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C), uric acid(UA), Apo A, Apo B, LP(a),blood routine, APN was measured in all cases, and detail data about gender, age, height,weight, smoking history, history of hypertension, and diabetes history was investigated.Statistical results was compared by statistical analysis in each group.Result:1 No statistical significance was noted in gender, BMI(P>0.05) in the general clinical data comparison of each group. Statistical significance in age, hypertension, diabetes,smoking history was observed in Medium and severe calcification VS no calcification and mild calcification group(P<0.05);2 Significant increased of serum TC, TG, LDL- C, UA, Apo B, Lp(a) was noticed in severe and moderate calcification group VS no calcification group(P<0.05), but significant reduction of serum HDL-C, Apo-A in moderately severe calcification group VS nocalcification group(P<0.05). And a significantly higher level of UA, Apo-B was observed in severe calcification group VS moderate calcification group(P<0.05), on the contrary,Apo-A is lower(P<0.05);3 Significant increased of neutrophil count and mononuclear cell count was noted in moderate, severe calcification groups VS no calcification, but lower hemoglobin(P<0.05);4 Significant decline of serum APN was observed in severe, moderate calcification group VS no calcification group(P<0.05). The same result was found in severe calcification group VS moderate calcification group. Indicating that serum APN concentration is gradually reduced varies with the degree of coronary artery calcification in aggravating;5 CAC multiariable Logistic regression analysis indicated that age, diabetes, smoking history, Apo-B, LP(a), UA is associated with CAC(P<0.05), which are the independent risk factors for CAC,but Apo-A, HDL-C, APN is negatively related to the CAC(P < 0.05),as the CAC protective factor.Conclusions:Serum adiponectin level is negatively related with the occurrence and development of CAC, which is protective factor. Endogenous human serum APN has effect of anti-inflammatory, inhibit coronary atherosclerosis and coronary artery calcification; Age,Diabetes, smoking history, TC, TG, LDL-C, Apo-B, Lp(a), UA were the risk factors for CAC. And age, diabetes, smoking history, Apo-B, Lp(a), UA were the independent risk factors for CAC, HDL-C and Apo-A are negatively related to the CAC, which are the protective factors.
