Construction And Practice Of Family Medicine Based Two-Way Referral Model In The Medical Consortium

Objective:To explore the primary medical care model which is in line with the situation of Zhejiang Province, We constructed and practiced family medicine based transfer model in the medical consortium for one year.Methods:This study retrospectively compared the operative data before and after set up of the discipline based medical model within the medical consortium among the hospital of Second Shaoxing Hospital and Ping Shui branch. The patients and staff satisfaction were also investigated.Result:1. The business volume, outpatient visits, hospitalization patients and the portion of the ailment of the Ping Shui branch were significantly increased after adopting the new model.2. After the establishment of the new model, the two-way transfer patients increased obviously, indicating that the patients are satisfied with the two-way transfer pattern.3. The patient and doctor satisfaction survey that they are both satisfied with the new model after establishing for one year (P< 0.001).Conclusion:1. The new model can improve the role of the family medical in medical practice.2. The new mode show the disciplinary advantages of the family medicine.3. The patients are satisfied with the new medical mode.
