The Discussion On The Role Of Lung Function In Early Diagnosis And Clinical Evaluation Of Chronic Obstructive Pulmonary Disease

Background:Chronic obstructive pulmonary disease is one of the four major chronic diseases who emphasized national prevention and treatment of the 13 th Five-Year plan for economic and social development of China.The main basis for its diagnosis and evaluation is the progressive decline in lung function.Global initiative for chronic obstructive lung diseases guidelines emphasize any patient who are considered to be suffering from COPD should be performed pulmonary function.Objective:Through the study of COPD prevalence,diagnosis rate,omission diagnose rate,the impact of smoking on lung function and the change of lung function when COPD combined with comorbidities,the role of pulmonary function in early diagnosis and clinical evaluation of COPD is discussed.Methods:Over 40-year-old 1393 participants were randomly collected from communities of Changchun.COPD questionnaire and lung function test are conducted in all participants.The questionnaire includes the basic information of participants,the risk factors of COPD,the past history and so on. Pulmonary function tests and quality control standards reference to guidelines for lung function tests in 2014.Results:1.We found 176 patients of COPD in this survey,the prevalence of COPD is12.64%.There are 160 patients missed diagnosis.The rate of missed diagnosis of COPD is 90.91%.There are 16 patients who had history diagnosis of COPD,the diagnosis rate of COPD is 9.09%.2.Smokers are linked to high prevalence of COPD(20.71%) compared with non-smokers(8.8%),the difference is statistical significantly(P<0.05). It shows a significant negative correlation between the smoke index and lung function,the higher the smoke index increases,the lower the lung function is.3.Comparing with the simple COPD group,the lung function of COPD combined with comorbidities group are obviously lower,the difference is statistical significantly(P<0.05).The lung function show no significant diference between the health group and non-COPD bearing other dieases group.4.Comparing with COPD combined with one comorbidity group,although the lung function of COPD combined with two comorbidities group are lower,but no significant difference is found.The lung function of COPD combined with ≥ 3comorbidities group are obviously lower than COPD combined with one comorbidity group,the difference is statistical significantly.5.Comparing with the simple COPD group,the lung function of COPD combined with asthma group are obviously lower except FVC%,the difference is statistical significantly.The lung function of COPD combined with cardiovascular disease group are obviously lower than the simple COPD group except PEF% and FEF75%,the difference is statistical significantly.There is no obvious difference in lung function between the COPD combined with diabetes mellitus group and the simple COPD group.6.Asthma and cardiovascular disease(hypertension、coronary heart disease) are the risk factors of COPD.Conclusion:1.The prevalence and the rate of missed diagnosis of COPD are higher in community residents 40 years of age or older in changchun.Lung function plays a vital role in early diagnosis of COPD.2.The prevalence of COPD in smokers is significantly higher than non-smokers.For patients with COPD, the greater amount of smoking,the more severe lung function damage.3.Compared with the simple COPD patients,lung function injury of the COPD combined with comorbidities patients is more serious, and with the increase in the number of comorbidities, the degree of decline in lung function also increases.4.Asthma and cardiovascular disease(hypertension,coronary heart disease) are the risk factors of COPD.
