Objective:This study aims to observe the polymorphism of MTHFR gene, analyze the relevancy between the two patterns of syndrome and the MTHFR genotype as well as their distribution in different genotypes through gene sequencing on the patients with the heart blood stasis symdrome and the sputum obstructing heart vessel symdrome of the thoracic obstruction, and further investigate the the role of the two TCM patterns of syndrome and MTHFR gene polymorphism in the evaluation of the prognosis of the unstable angina pectoris.Methods:By means of the prospective investigation method and according to the diagnosis criteria of unstable angina pectoris, the included subjects were the patients hospitalized in Department of Cardiology of Affiliated Hospital of Nanjing University of Traditional Chinese Medicine (Jiangsu Provincial Hospital of Traditional Chinese Medicine) to receive the MTHFR gene sequencing who conformed to the western medicine diagnosis criteria of unstable angina pectoris and TCM differentiation of thoracic obstruction, the heart blood stasis symdrome and the sputum obstructing heart vessel symdrome and who voluntarily participated in this clinical study, and the number of cases was 209. The general materials and the relevant materials such as adverse cardiovascular events as indicated in the follow-up visits half a year after the discharge were collected to establish a database, and the SPSS 19.0 statistical software was utilized to analyze the relevancy of the heart blood stasis symdrome and the sputum obstructing heart vessel symdrome of thoracic obstruction with MTHFR genotype.Study Results:1. Among the 209 patients,98 was the heart blood stasis symdrome (as for the genotype 40 cases of CC,46 cases of CT and 12 cases of TT); 111 was the type of the sputum obstructing heart vessel (as for the genotype,27 cases of CC,64 cases of CT and 20 cases of TT).2. The heart blood stasis symdrome and the sputum obstructing heart vessel symdrome were related with the plasma homocysteine level and MTHFR genotype (P< 0.05), and MTHFR genotype was significantly related with the plasma homocysteine level (P< 0.05).3. The two TCM patterns of syndrome are distributed in CC genotypes statistically significant (P<0.05); The two TCM patterns of syndrome distribution in CT, TT genotypes were not statistically different (P> 0.05).4. The study did not conclude that two TCM patterns of syndrome, the MTHFR genotype and the plasma homocysteine level were the independent correlation factors of MACEs (P> 0.05).Conclusion:The heart blood stasis symdrome and the sputum obstructing heart vessel symdrome of the thoracic obstruction are associated with the MTHFR genotype. In the two TCM patterns of syndrome, CC genotype tend to be expressed in the pattern of the heart blood stasis symdrome, and there was no statistical difference between CT and TT genotypes in terms of the expression in the two TCM patterns of syndrome. The two TCM patterns of syndrome, the MTHFR genotype and the homocysteine level cannot be still used to evaluate the incience rate of cardiovascular adverse events within a short period of time.
