Objective: To investigate the relationship between different levels of homocysteine in serum and coronary atherosclerotic lesions, their severity and plaque stability. Methods: From September 2017 to September 2019,a total of 220 patients with coronary angiography who have met the selection criteria and have undergone coronary angiography in the Department of Cardiovascular Medicine of the People's Hospital of Ningxia Hui Autonomous Region were selected.Collect general clinical data, serum homocysteine concentration, and coronary angiography results of these patients.According to the results of coronary angiography, they were divided into non-coronary heart disease group(91 cases)and coronary heart disease group(129 cases). The coronary heart disease group was divided into acute coronary syndrome group(ACS group)and Stable coronary atherosclerotic heart disease group(SCAD group). The coronary heart disease group calculated the Gensini score based on the results of coronary angiography. The general clinical data and homocysteine concentration of the coronary heart disease group and the non-coronary heart disease group were compared, and the factors with statistical differences in the general data were compared with homocysteine for multivariate logistic regression analysis. The general clinical data and homocysteine levels between the two subgroups of coronary heart disease were compared, and the factors with statistical differences in the general data were analyzed by multivariate logistic regression. The correlation between homocysteine concentration and Gensini score was analyzed. Results: 1.There were statistical differences between the coronary heart disease group and the no coronary heart disease group in gender, smoking history,Hcy,total cholesterol, uric acid,BMI(P <0.05);2.Multiple logistic regression analysis showed that Hcy,total cholesterol, and gender are related to the incidence of coronary heart disease and are independent risk factors for coronary heart disease;3.The statistical differences between the ACS group and the SCAD group were: smoking history,Gensini score, total cholesterol(P<0.05),and there was no difference in serum Hcy levels between the two groups(P> 0.05);4.Multiple logistic regression analysis showed that Gensini score and total cholesterol were related to the stability of coronary atherosclerotic plaque and were independent risk factors;5.Serum Hcy level was low positively correlated with the Gensini score representing the severity of coronary atherosclerosis and had significant statistical significance. Conclusions: 1.Serum Hcy is positively correlated with the occurrence of coronary atherosclerotic lesions, which may be its risk factor, and serum Hcy level is positively correlated with the Gensini score, indicating that Hcy may be related to the severity of coronary atherosclerotic lesions;2.There was no difference in serum Hcy levels between the ACS group and the SCAD group, suggesting that serum Hcy levels may not be related to the stability of coronary atherosclerotic plaques, but total cholesterol level may be a risk factor affecting plaque stability in coronary atherosclerotic lesions.
