This Study Of The Effect Of Nourishing Qi, Warming Yang And Dredging Collaterals On The Prevention And Treatment Of Neurotoxicity Induced By Oxaliplatin

ObjectiveThis study that is prospective, randomized and controlled was aimed to observed the effect of Nourishing qi, warming yang and dredging collaterals on the prevention and treatment of neurotoxicity induced by oxaliplatin, and the correlations between nerve conduction velocity and neurotoxicity.MethodForty patients diagnosed with gastrointestinal malignant tumors from Department of oncology, the First Affiliated Hospital of Guangzhou University of Chinese Medicine, being treated with chemotherapy containing oxaliplatin, were randomly divided into treatment group and control group. Two groups accepted dialectical use by tranditional Chinese medicine., the treatment group with five drus Decoction, the control group with decoction of four mild drugs. In 24-48 hours before the first chemotherapy, chemotherapy in 2,4,6 history (3 weeks,2 weeks, observe the 3,6,8 scheme), after 24-48 hours for the electromyography recording on the sensory and motor nerve conduction velocity, grade of neurotoxicity, and recorded before chemotherapy, the age, surgery treatment condition, various time points of carcinoembryonic antigen, carbohydrate antigen 125, carbohydrate antigen 199, total number of WBC, neutrophil, hemoglobin, platelet, the numerical cumulative dose of oxaliplatin, and traditional Chinese medicine syndrome score, carries on the comparison to the above indicators, analyzing the relevance of what is happening and neurotoxicity.ResultsThere were no significant differences on hemoglobin, platelet, cumulative dose in 4 time points between the two groups(P> 0.05); the WBC in chemotherapy 6,4 (or 6,8) differ (P< 0.05), NEU in 4 (or 6) differ (P< 0.05). On the incidence of nerve toxicity two groups had no statistical difference (P> 0.05), which did not appear the level IV neurotoxicity. Two groups on TCM syndrome score of three time points after chemotherapy comparing to that before chemotherapy had no statistical difference (P> 0.05). The treatment group not differed from the contronl group in sensory and motor nerve conduction velocity at the same time (P> 0.05). Before and after treatment nerve conduction velocity of two groups showed a downward trend(P>0.05). Correlation existed between sensory nerve conduction velocity of upper limb and neurotoxicity.ConclusionNourishing qi, warming yang and dredging collaterals based on five drus Decoction had certain curative effect in prevention of neurotoxicity induced by oxaliplatin, which was no significant difference from invigorating spleen and benefiting qi. The chronic neural toxicity was associated with neural electrophysiological changes, mainly reflecting in sensory nerve conduction velocity of upper limb.Nerve conduction velocity and chemotherapy cycle changed negatively.
