| Objective:To investigate the influence on the different second stage of labor duration of birth process.Methods:Data were from retrospective record reviews of nulliparous women delivering a singleton infant in cephalic presentation at term time between January 2014 and May 2015 in our hospital,whose second stage duration beyond 2 hours,without the use of epidural analgesia.127 cases met the conditions,as the study group.they were divided into 2 groups on second stage duration(t):Study Group 1 consisted 110 women with 2h≤t<3h, Study Group 2 consisted 17 women with t≥3h.We selected randomly 106 nulliparous women with t<2h over the same period,as the Control Group.Maternal and neonatal outcomes of Study group 1 and 2 were compared with those of the Control group, respectively.Results:(1)Study Group 1 had higher rates of forceps delivery and episiotomy than those of Control Group(P<0.05).The rates of postpartum hemorrhage, postpartum fever, urinary retention, shoulder dystocia, neonatal academia, neonatal asphyxia, admission to the neonatology department were not significantly increased(P>0.05).(2) Study Group 2 had higher rates of forceps delivery, episiotomy and postpartum hemorrhage than those of Control Group(P<0.05).The rates of postpartum fever, urinary retention, shoulder dystocia, neonatal academia, neonatal asphyxia,admission to the neonatology department were not significantly increased(P>0.05).Conclusion:There is no increase in severe adverse maternal and neonatal outcomes with second stages beyond 2 hours,and less than 3hours. Maternal morbidities are significantly increased with second stages beyond 3 hours. |
