The Clinical Study Of The Treatment Of Chronic Cor Pulmonale Decompensated Period Combined With Hepatic Congestion Used Xiaochaihu Tang Plus Xiaqishengxue Tang Jiajian

Objective: Through clinical curative effect of observation which the treatment of chronic cor pulmonale decompensated period combined with hepatic congestion used Xiaochaihu Tang Plus Xiaqishengxue Tang Jiajian,to explore a more perfect treatment regimen which combined Chinese and Western medicine in the treatment of chronic cor pulmonale decompensated period combined with hepatic congestion. Method:Selected 60 patients consistent with the chronic cor pulmonale decompensated period with hepatic congestion and the Chinese medicine dialectical is Tanyinzufei and Luoqibuhe type, according to the order in hospital, Were divided into the joint group and the control group randomly, the control group of 27 cases, the joint group of 33 patients, two groups in the process of observation are not out and fall off.The control group of conventional western medicine therapy(infection control, remove the airway spasm, expectorant, correct water electrolyte and acid-base imbalance, short-range using glucocorticoid, nutrition support treatment), the joint group's foundation treatment is the same with the control group, what's more, the joint group must take Xiaochaihu Tang Plus Xiaqishengxue Tang Jiajian under a 100 ml, three times a day, 1 dose of 1 day.Treatment for 7 days.Records of two groups of patients' age, gender, course of the disease, the TCM syndrome integral which were before and after treatment including(1 day, 3 days, 7 days), pulmonary function, NT-pro BNP, arterial blood gas, hs-CRP, LVEF, liver and renal function examination index, using the statistical software SPSS 13.0 analysis of the data. Results:The two groups before treatment base case there was no significant difference.(1)Joint group clinical curative effect(93.9%) was significantly higher than the control group(74.1%).(2)There were differences in the joint group compared with the control group(P < 0.05) in the improvement of TCM syndrome score which equaled the integral before treatment subtract the integral after treatment.(3)Ventilation index comparison: FEV1, FEV1 / FVC %, FEV1 % of expected values than before treatment were improved(P < 0.01), but the joint group increased more significantly than control group(P < 0.05), the difference was statistically significant.(4) Blood gas analysis comparison: Pa CO2, Pa O2 which in combined group and control group after treatment are difference compared with before treatment(P < 0.01), compared the two group after treatment,Pa CO2, Pa O2 no difference(P > 0.05).(5)NT-pro BNP was decreased(P < 0.05) in the two groups after treatment, NT-pro BNP decline than the control group significantly(P < 0.01) in the joint group after treatment, the difference had statistic significance.(6)The hs CRP score in the two groups after treatment was decreased(P < 0.01), the joint group of hs CRP score decreased after treatment than the control group significantly(P<0.05), the difference had statistic significance.(7)The joint group's LVEF value in Heart colour doppler checking after treatment is higher than its before treatment, the difference had statistic significance(P<0.05), the increased LVEF value in the control group after treatment was not obvious(P>0.05), the difference was not statistically significant.(8)Security comparison: The AST, ALT, BUN and Cr in the two groups after treatment no difference compared with before treatment(P > 0.5).Conclusion:the treatment of chronic cor pulmonale decompensated period combined with hepatic congestion used Xiaochaihu Tang Plus Xiaqishengxue Tang Jiajian have six merits to the patient:(1)To improve clinical curative effect;(2)Improve ventilation function;(3)Correcting heart failure;(4)Reduce inflammation response;(5)Improve cardiac ejection function;(6)Less effect on safety indexes such as liver and kidney function.
