Objective: Diabetes mellitus is a group of metabolic diseases characterized by sugar and lipid metabolism disorders.High blood sugar in diabetic patients can lead to various tissues,especially in the eyes,heart,kidney,blood vessels,nerves and other organs of chronic damage,dysfunction.Diabetic foot ulcers is one of the major health problems of our time.Diabetic patients with peripheral neuropathy and peripheral vascular disease combined with high mechanical pressure,can cause the foot soft tissue and bone and joint system damage,resulting in a series of problems,from mild neurological symptoms to severe ulcers,infection,vascular disease,Charcot disease and neuropathy.Diabetic foot traditional treatment methods: the long-term effect of traditional drug therapy,vascular bypass surgery and interventional operation is not ideal,especially lower extremity arterial outflow tract occlusion and the lack of compensatory collateral patients,using any means it is difficult to make the recanalization,disabling foot ulcers or gangrene and amputation surgery causing,some patients after amputation is life-threatening.Data show that about 1/4 of patients with diabetes in our country with diabetes.Late in the treatment of patients with extensive infection,amputation rate and high mortality.VAC refers to using containing drainage tube of polyvinyl alcohol water seaweed salt foam dressing,to cover or fill the wound of skin and soft tissue defect,then by biological semipermeable membrane on the closed the become a confined space,finally,the drainage tube communicated with a negative pressure source,through a controlled negative to promote wound healing of a new treatment method.To investigate the clinical efficacy of VAC in treating diabetic foot ulcers.Methods:1 68 patients with diabetic foot ulcers were treated through surgical debridement,and those were divided into two groups,according to the Wagner of each grade and the voluntary of the patients were randomly divided into treatment group and control group,each group of patients were 34 cases.2 The treatment group was treated with conventional dressing change,local secretion germiculture is electronegative and VAC treatment.Follow,free skin graft or flap transfer,closed wound.3 The control group was treated with routine traditional dressing change,and no necrotic tissue was found in the wound after operation.And,free skin graft or flap transfer,closed wound.Results: Two groups of patients after treatment,the number of dressing change,wound healing time,hospital costs,amputation rate,limb salvage rate and the recurrence rate was significantly better than the control group patients were significantly better than the control group,Through the comparison between the 2 groups,the expression of statistical significance(P<0.05).Conclusion:In the treatment of diabetic foot patients with VAC technology,can shorten the recovery time,avoid cross infection,reduce the use of clinical antibiotics,reduce the number of dressing change and the pain of patients,shorten the hospitalization time and reduce the cost of hospitalization.It is a safe and effective method to promote the healing of diabetic foot ulcers. |
