Objective: Our study retrospectively investigated patients with acute lacunar stroke in our hospital.By analyzing basic information and related factors of these patients to identify acute lacunar stroke risk factors of early neurologic deterioration.,in order to provide theoretical basis for clinical prevention and treatment in the future.Methods: We collected 131 patients in department of neurology from January 1,2014 to December 31,2015 who were diagnosed acute lacunar stroke.We divided these patients into the progressive group(PLS)and the non-progressive group(Non-PLS).Caculate the incidence of PLS in hospitalized patients who were diagnosed acute lacunar infarction.In univariate analysis,variables such as general condition,laboratory indexes and imaging characteristics were compared between the two groups.To determine the possible contribution factors for PLS,multivariate logistic regression analysis were done using variables with P values<0.05 in univariate analysis.Statistical significance was established as P < 0.05 and 95% CI contains no 1.Results:1.In our study,we selected 131 patients together,there were 35 patients in PLS group and 96 patients in Non-PLS group,the frequency of PLS is 26.72%,23(65.71%)patients occurred disease progression during the first 48 hours after stroke onset.2.Related factors analysis:(1)PLS group compared with Non-PLS group,there were no statistical difference in mean age gender,initial NHISS score,initial diastolic blood pressure at admition,diabetes mellitus,hyperthermia,leukocyte count,urine specific gravity >1.010,the interval between onset to magnetic resonance and infarction with slice number(P>0.05).(2)PLS group compared with Non-PLS group,there were statistical difference in history of smoking and initial systolic blood pressure admition(P<0.05);there were significant statistical difference in hypertension,initial systolic blood pressure at admition,hyperglycemia,hypertriglyceridemia,maximum diameter of lesion on DWI,parent artery stenosis and the different infarction(P<0.01).(3)Multivariate logistic regression analysisMultivariate logistic regression analysis showed that,the hypertension(OR=4.208,95%CI 1.283-13.796),hyperglycemia(OR=3.926,95%CI 1.073-14.363),hypertriglyceridemia(OR=4.058,95%CI 1.263-13.037),parent artery stenosis(OR=6.839,95%CI 2.305-20.290),the infarction in the performating artery territory of vertebral artery(OR=8.923,95%CI 1.488-53.523)were independent risk factors of PLS.Conclusion: 1.In our study,the hypertension,hyperglycemia,hypertriglyceridemia,parent artery stenosis,the infarction in performating artery territory of vertebral artery were independent risk factors of PLS.2.We should pay more attention on the physical-chemical indexes and imaging characteristics of hospitalized patients in order to take effective,active prevention and treatment in the early time for disease and prevent the disease continuing to deteriorate.
