Objective:The aim is to investigate the prognostic factors of stage ? colon cancer by retrospective study and provide basis to the prognosis evaluation about stage ? colon cancer.Materials and Methods:1.We enrolled 175 stage ? colon cancer patients with pathology who underwent curative surgery in the First Affiliated hospital of Dalian Medical University from January 2008 to January 2012.All of the patients have complete clinical data.There are 129 patients left through a inclusion criteria.We collect the patients' clinical data,including age,gender,tumor location,histology,differentiation of tumor,lymph nodes harvested,tumor invasion depth,preoperative serum CEA level,preoperative serum CA19-9 level,preoperative blood platelet count,preoperative hemoglobin,obstruction,adjuvant chemotherapy.129 patients were retrospectively analyzed to study this factors in stage ? colon cancer and the 3-year disease free survival rate and 3-year overall survival rate correlation.For the purpose of determining the factors associated with survival.2.As adjuvant chemotherapy,5-FU based chemotherapy was administered for 6 months.The adjuvant chemotherapy regimens included FOLFOX(L-OHP+5-FU+LV)?XELOX(L-OHP+CAP)?5-FU/LV and CAP,several patients selected oxaliplatin combined carmofur,SOX(L-OHP+ S1).The diagnosis of anemia used domestic blood disease diagnostic and curative Standard;Tumor stage according to the AJCC-7 edition staging criteria for colon cancer;3.Follow-up:Follow-up of the patients has been regularly updated by telephone interviews yearly or consulted electronic medical records.The enrolled patients were followed up at 3-month intervals for an initial 2 years and 6-month intervals thereafter till 5 years.All of the enrolled patients were also followed up on until death or on January 1,2016.4.All analyses were carried out with SPSS version 17.0.Univariate analysis of the Chi-Square test and Kaplan-Meir method was used.Survival curves were generated by the Kaplan-Meier method.Log-rank test was performed to establish the statistical significance.Multivariate analysis was performed using the Cox proportional hazard model.Statistical significance was defined at P<0.05.Results:1.A total of 129 stage ? colon cancer patients were involved for the study.Up to January 1,2016,tumor recurrence arised in 18 patients(13.9%),11(8.5%)patients died.the 3-year disease-free survival rate and overall survival rate of this cohort were 89.2%and 93.4%,respectively.2.In univariate analysis,the following variables were significantly associated with the poor 3 year disease-free survival rate :T4 depth of tumor invasion(P=0.017),preoperative serum CEA>3.4ng/ml(P=0.020),the severity of anemia(P =0.003).but patients 'age,gender tumor location,histology,differentiation of tumor,preoperative serum CA19-9 level,preoperative blood platelet count,lymph nodes harvested,obstruction,adjuvant chemotherapy are the factors with have no significant effect on 3 year disease-free survival rate.The following variables were significantly associated with adverse 3-year overall survival rate: T4 depth of tumor invasion(P=0.031),preoperative serum CEA>3.4ng/ml(P=0.016),the severity of anemia(P=0.025),less than 12 lymph nodes(P = 0.041).while patients ' age,gender,tumor location,histology,differentiation of tumor,preoperative serum CA19-9 level,preoperative blood platelet count,obstruction,adjuvant chemotherapy are the factors with have no significant effect on 3-year overall survival rate.3.In multivariate analysis,T4 depth of tumor invasion(P=0.004),preoperative serum CEA>3.4ng/ml(P=0.016),the severity of anemia(P = 0.009)were independentprognostic factors for the 3-year disease-free survival rate,while T4 depth of tumor invasion(P=0.004),the severity of anemia(P = 0.020),less than 12 lymph nodes(P=0.019)were independent prognostic factors for the 3-year overall survival rate.Conclusion:1.T4 invasion,preoperative serum CEA>3.4ng/ml and the severity of anemia may correlated the 3 year disease-free survival rate of stage ? colon cancer patients.2.T4 invasion,the severity of anemia and less than 12 lymph nodes may correlated the 3 year overall survival rate of stage ? colon cancer patients.3.T4 invasion,the severity of anemia may be an independent prognostic factor for stage ? colon cancer.
