Efficacy Of Laparoscopic Complete Mesocolic Excision In Radical Right Hemicolectomy For Right Colon Cancer

Objective:To investigate the efficacy between laparoscopic and open complete mesocolic excision(CME)in radical right hemicolectomy for right colon cancer.Methods:75 patients underwent laparoscopic CME and 75 patients underwent open CME for right-sided colon cancer between January 2012 and January 2015 at our hospital.Operation time,intraoperative blood loss,postoperative exhaust and defecation time,length of hospital stay,surgical specimen section degree,number of removed lymph nodes in different TNM,number of positive lymph nodes and postoperative complications were compared between the two groups.Follow-up of 3years,compare two groups of 3-year local recurrence rates,3-year overall survival rates and 3-year disease-free survial rates.Results:Laparoscopic group were significantly lower than open group in intraoperative blood loss,postoperative exhaust and defecationtime,length of hospital stay(P<0.001);There were no significant differences in operation time and postoperative complications between the two groups(P>0.05).The surgical specimen section degree in laparoscopic group were better than those in open group(P<0.05),The numbers of lymph nodes retrieved in TNM?,?and? in laparoscopic group was more compared with the open group(P<0.05),but the number of positive lymph nodes in both groups were not significantly different(P>0.05).There were 2.8% and4.2% in 3-year local recurrence rates,88.9% and 84.8% in 3-year overall survival rates,83.1% and 79.2% in 3-year disease-free survival rates between the two groups with no significa-nt differences(P>0.05).Conclusion:Laparoscopic CME can achieve the same effect as open CME in oncologic radical resection for right-sided colon cancer and reduce intraoperative blood loss,ensure the integrity of the mesentery,retrieve more lymph nodes,get higher quality surgical specimens.
