Objective:To observe the influence of DJC which has the function of Yiqi Yangyin Huoxue on TIMP-2、hs-CRP in serum of patients with type 2 diabetes and the expression of NF- nuclear factor kappa B in the kidney of diabetic rats.To investigate the correlation between inflammatory cytokines and type 2 diabetes and describe the effect of DJC on inflammatory markers in vascular disease in patients with type 2 diabetes,evaluating the mechanism of action of DJC in preventing and delaying the chronic complications of diabetes.Method:1.Clinical research60 patients with type 2 diabetes were randomly divided into two groups,each group has 30 cases.The control group was treated with Western Medicine,the treatment group was treated with western medicine therapy combined with Danzhijiangtang Capsule(3times a day for each 5),and the course of treatment was 12 weeks in each group.All cases were Recorded FPG, Hb A1 c, blood lipid, creatinine and other indicators of patients before and after treatment,and detecting the level of TIMP-2 and hs-CRP.Another 30 cases of healthy persons were collected as normal group.2.Experimental research After one week of adaptive feeding, 55 healthy male rats were randomly divided into two groups, 9 as the normal control group, and the rest as the experimental group.Normal control group was fed with basic feed until the end of the experiment;The experimental group was fed with high fat diet.Intraperitoneal injection of STZ of 35mg/kg after 4 weeks to establish the animal model of diabetes.41 rats were randomly divided into 4 groups in addition to the 5 rats which was failed to build model:11 rats in model group, 10 rats in low dose group of DJC, 10 rats in high dose group of DJC, 10 rats in pioglitazone group.High does group of DJC and low does group of DJC were treated with the dose of 1.08g/(kg?d),0.54g/(kg?d) by gavage,pioglitazone group was treated with the dose of 10mg/(kg?d) by gavage, the normal control group and model group were treated with physiological saline of5ml/(kg ? d) by gavage, high fat diet for 8 weeks until the end of experiment.Results:1.Clinical research results:The clinical results showed that the total effective rate of the treatment group and the control group were 81.5% and 50% respectively after 12 weeks of treatment,the curative effect of treatment group was better than that of control group.Compared with the same group before treatment, the TCM syndrome scores of the control group and the treatment group were all improved significantly after treatment(P<0.01),and TCM syndrome score in treatment group was lower than that in control group after treatment(P<0.05).Compared with the control group, the expression of TIMP-2 and hs-CRP of the treatment group decreased significantly(P<0.01).TCM syndrome points,FPG, Hb A1 c,triglyceride and total cholesterol, creatinine, urea nitrogen were decreased obviously than that before treatment in treatment group and control group after treatment,the difference was significant(P < 0.05 or P < 0.01).2.Experimental results Animal experimental results show that compared with the normal group,the blood glucose of model group, DJC high and low dose group, pioglitazone group was increased significantly after modeling(P < 0.01).After 2 weeks of treatment, the blood glucose of high dose group of DJC and low dose group of DJC reduced significantly(P<0.01).After 4-8 weeks,the blood glucose of each group was decreased significantly compared to the model group(P<0.01).The study showed that the expression of NF-nuclear factor kappa B in model group was significantly increased,especially in the renal tubular and interstitial cells; the expression of NF-nuclear factor kappa B in rat kidney in normal control group has no expression or express weak;the expression of NF kappa B in DJC high dose group and DJC low dose group in renal tissues was decreased significantly compared with the model group(P<0.01);The expression of NF- k B in pioglitazone group was less than model group,most showed weakly positive expression.The positive index of NF kappa B in DJC high dose group and DJC low dose group was decreased significantly after treatment, there was a significant difference compared with the model group(P<0.01).Conclusion:DJC which has the function of yiqiyangyinhuoxue can inhibit strong reaction of TIMP-2 and NF-k B, hs-CRP and other inflammatory factors.The mechanism may be related to reduced blood sugar and high expression of inflammatory factors, regulated the body’s immune balance and so on.
