| Objective:The intention of the subject is to evaluate the therapeutic effect of electroacupuncture in the treatment of serious functional constipation and its possible mechanism.We hope it can provide the objective basis for clinical application.Method:60 patients with Serious FC From Wuhan Integrated Traditional Chinese and Western Medicine Hospital were divided into two groups,30 patients respectively,the patients of treatment group were given deep needing Tianshu,Fujie,Shangjuxu and the control group received shallow needling non acupoint nearby Tianshu,Fujie,Shangjuxu for EA or sham EA treatment.A course of treatment was one week,total of 8courses.The patients received treatment of 5 times a week in the first 2 weeks and 3 times a week in the following 6 weeks,each time for 30 minutes in total Twenty-eigh times.Before and after treatment,the clinical curative effect and the number of CSBM,spontaneous bowel movements,BRISTOL fecal character score,degree of difficulty in defecation and PAC-QOL scale score of patients were observed and changes of plasma levelsof serotonin and nitric oxide synthase of patientsin were tested.We made comparative analysis of the data.Result:1.Comprehensive clinical curative effect comparison:After the treatment,30 cases were completed successfully in each group,the total effective rate of EA was 93.33% with sham EA66.67%;the difference of the clinical comprehensive efficacy of the two groups was statistically significant(P<0.05),The better effection was in the The curative group.2.The symptom indicators comparison:No statistically significant differences of each symptom index hanppened in two groups before treatment(P>0.05),full of comparable in two groups;But after treatment,The difference was meaningful Within each group(P <0.05),showing that both treatment can improve the symptoms index of constipation;there were significant differences between the EA and sham EA group(P<0.05),The curative effect is more obvious to the improvement of the constipation symptoms.3.Patients with PAC-QOL scale evaluation:Before treatment,PAC-QOL scores have no obvious difference in the two groups(P>0.05),was comparable;after treatment,PAC-QOL scale scores are lower than before,but the EA treatment was more better(P<0.05).4.The content of plasma NOS and 5-HT : There was no significant difference in two groups before treatment(P>0.05);After treatment,the NOS levels of the two groups decreased and the 5-HT increased significantly(P<0.05),but the difference in EA group was more significant(P<0.05).5.Safety index evaluation:There was no serious adversereactions occuring in the two groups of patients with FC during the process of receiving treatment.All the patients can reseive the treatment,and none of them suspended the experiment because of serious adverse reactions.Conclusion:1.Electro acupuncture is an effective and safe method in the treatment of severe serious FC,which can not only improve the symptoms of constipation,but also improve the quality of life of patients.So,the method of EA treatment for functional constipation is worth of clinical application.2.The curative treatment of serious FC in general curative effect,on the improvement of the symptoms and life quality improvement and so on are better than control group obviously,showing that electric acupuncture group is better than that of sham electric acupuncture group.3.The mechanism of electric acupuncture treatment of serious FC is full of complexity.We speculated that one of the effective mechanisms of electroacupuncture in the treatment of serious FC may be by regulating the level of the neurotrans mitter NOS and 5-HT in the patient’s body. |
