| Objective: 1.To observe the effectiveness and safety of Xiangsha Wen Zhong Tang in Treating Chronic Atrophic Gastritis(CAG)due to spleen and stomach qi(Yang)deficiency type.Xiangsha Wen Zhong Tang is one of the the behalf prescriptions of Professor Li Zhenhua,s Theory of Spleen and Stomach liver zang and fu syndrome differentiation.2.To evaluate the advantage of the characteristic method of differentiation over the general method of differentiation.3.To investigate the short term clinical curative effect and Partial mechanism of action of Xiangsha Wenzhong Tang.Methods:The 200 patients with CAG,according to the diagnostic and syndrome differentiation standard of traditional Chinese medicine and diagnostic standard of Western Medicine were divided into treatment group(99 cases)and control group(101cases).The treatment group were treated by Spleen and Stomach liver zang and fu syndrome differentiation,and the control group were treated with the general method of differentiation.A four-drug regimen for 14 days were given with the patients were Hp infectors.1 month after the withdrawal of drugs the patients can retest the C13 breath examination.No matter the results of the examination was positive or negative,the experiment should continue to be carried out.Each patient was given continuous medication for 6 months,without other medicine given during this period.The score of main symptoms,secondary symptoms and symptoms in Chinese Medicine before and after treatment was calculated.To observe the reducing and disappearing time of the main symptoms,the change of gastric mucosal pathology of the two groups before and after treatment,and the data were statistically analyed.Results:(1)After treatment,the TCM main symptoms and secondary symptoms,symptom scores of the two groups were both significantly reduced compared with before treatment,the difference was significant(P<0.05),suggesting that these two methods can improve the TCM symptoms of patients with CAG.The reducing and disappearing time of the main symptoms before and after treatment of the treatment group were less than that of the control group,and the difference was significant(P<0.05),suggesting that the treatment group was superior to the control group in reducing and improving the main symptoms of CAG patients,especially in the symptoms of epigastric fullness,epigastric pain and difficult in digesting after eatting(P<0.05).The symptomatic cure rate of treatment group was 67.8%,higher than the control group(P<0.05);the total effective rate was 94.9%,higher than the control group(P>0.05).(2)The cure rate of gastric mucosal atrophy in treatment group was 67.8%,higher than that in the control group 53.5%,the total effective rate was 73.9%,higher than that of the control group 42.6%(P<0.05).(3)The cure rate of intestinal metaplasia of gastric mucosa in the treatment group was 67.8%,higher than that of the control group 53.5%(P>0.05).The total effective rate was 47.9%,higher than that of the control group 40.7%(P>0.05).(4)In all included 200 cases,there were a total of 22 cases gastric mucosa dysplasia,due to the small sample size,the statistical results may lack repeatability.So it was not described in the article.(5)In the course of this study,no obvious adverse reactions were found.Conclusions:(1)Professor Li Zhenhua,s Theory of Spleen and Stomach liver zang and fu syndrome differentiation,can significantly improve the chronic atrophic gastritis of Spleen Stomach Qi(Yang)deficiency patients with general situation,main symptoms,clinical curative effect.Its clinical efficacy was significantly better than that of the control group.(2)Professor Li Zhenhua,s Theory of Spleen and Stomach liver zang and fu syndrome differentiation,can be significantly improved and reversal CAG patients with spleen and stomach qi(Yang)deficiency type in gastric mucosa of patients with atrophic,the overall effect is better than the general dialectical method.(3)The two types of syndrome differentiation can both improve and reverse the gastric mucosal atrophy of the CAG patients,but the date was no significantly difference.(4)Professor Li Zhenhua,s Theory of Spleen and Stomach liver zang and fu syndrome differentiation has safe and reliable curative effect,and can be widely used in clinical.(5)The short term effect of Xiangsha Wenzhong Decoction is exact,and its mechanism may be protect and repair the gastric mucosa,promote gastric emptying,alleviate the clinical symptoms,anti-inflammatory,enhance the immune function,improve circulation,and evently inhibit atrophy. |
