Study On Farmers Medical Security Under The Livelihood Capital Perspective

Rural medical security system is always a backward state of development in the construction of social public service. But it has played an indispensable role in the national health service. Because of the exposure of health risks, and the rise of medical expenses, and the backward of rural medical service, the phenomenon of rural poverty caused by disease is increasing, which becomes an important factor to hinder a well-off society build in an all-round way in China. To solve the medical "difficulty" and "expensive" problem, the government is not only to speed up the construction of rural medical security and improve the effective supply of medical products and services, and also to improve farmers’ living ability based on the "people-oriented" principle for improving the farmers’ effective demand of medical products and services.This paper follows "problem, literature review, logic theory, empirical analysis,policy refinement reconstruction", and studies the problems in four aspects on the farmers’ medical security problems under the livelihood capital perspective. Firstly, according to the theory of DFID sustainable livelihoods theory and health economics etc, we summarize the mechanism between farmers’ livelihood capital and medical security.Secondly, we study the present situation and problems of rural medical security in the Southeast District in Chongqing. Thirdly, with correlation analysis, variance analysis and Logistics regression model, we study the influence of farmers’ livelihood capital on the ability of medical support, on the participation willingness of the new rural cooperative medical system, and on the evaluation on rural public health service. Lastly,some suggestions are provided.The main conclusions of this study are as follows:(1) Sustainable livelihoods and rural medical security are a two-way connection. Sustainable livelihoods can improve farmers’ medical security ability, and meanwhile the ability can improve the livelihood capital and reduce health risks.(2) Livelihood capital has the influence on the ability of farmers’ medical support. The living of farmers in the southeast of Chongqing is fragile,and fragile livelihood capital leads to the low per capita health expenditure and the relatively high medical expenditure rate. Farmers’ livelihood capital has the influence on the ability of medical support, on the participation willingness of the new rural cooperative medical system, and on the evaluation on rural public health service,especially the human capital.(3)Based on the sustainable livelihoods framework, the government should improve the vulnerability context in order to reduce farmers’ health risks, and improve farmers’ livelihood capital to improve their medical security ability,and improve the medical security policy and rural public health service system to improve the supply level of rural medical and health. Due to the similar issue of farmers’ medical, the standard design of rural medical security system in the framework of livelihood capital, and the typical case of the southeast of Chongqing, the conclusions of this study have certain application and extension value on the government work.
