Objective:The purpose of the study was to research the clinical applicability of measuring serum gastrin-17(G-17) combined with pepsinogen(PGⅠ、PGⅡ、PGR) as screening tests for gastric cancer.Materials and methods:125 cases with high risk in gastric cancer were collected from our digestive department and outpatient service in the NO.1 hospital of Shanxi medical university from April 2015 to April 2016. The serum Gastrin-17, and pepsinogen PGⅠ, PGⅡ, and the ratio of PGⅠand PGⅡ(PGR)was tested.G-17≤1pmol/L or G-17≥15pmol/L was defined as G17(+). PGⅠ≤ 70ng/ml and PGR ≤ 7 as PG(+). According to the standard, the groups were divided into four groups, A、B、C and D.Group A was G-17(-) and PG(-),Group B was G-17(+) and PG(-), Group C was G-17(-) and PG(+) and Group D was G-17(+) and PG(+). At the same time, all cases was attribute to gastric cancer or non-cancer group on the bases of endoscopic biopsy.Compare the proportion of gastric cancer and explore the value of the combined test in gastric cancer screening.The results:1. The proportion of gastric cancer in group A was 6.6%, B 60%, C 40% and D 73.7%.There was significance in four groups(P<0.001); also between the group A with B, C,and D(P<0.05);There was no significance in statistics between the group B with C,and D(P>0.05);The proportion of gastric cancer in group D was significantly higher than theothers.2. The cut-off values of gastrin was G-17≤ 1pmol/L or G-17≥15pmol/L. And pepsinogen was PGⅠ≤ 70ng/ml and PGⅠ/PGⅡ≤7.0. The sensitivity of G-17 was 70.7%,and the specificity 82.1%. The sensitivity of PG was 53.7%, and the specificity 79.8%. The sensitivity of combined G-17 with PG was 34.1% and the specificity was 94%. Due to lack of prevalence of this area, the positive predictive value and the negative predictive value could not be calculated.Conclusion:In the research of measuring serum gastrin-17(G-17) combined with pepsinogen(PGⅠ、PGⅡ、PGR) as screening tests for gastric cancer, the cut-off values of gastrin was G-17≤ 1pmol/L or G-17≥15pmol/L. And pepsinogen was PGⅠ≤ 70ng/ml and PGⅠ/PGⅡ≤7.0. In the non-gastric cancer group, the probability of the test negative was90.4%. The false positive rate was 6%. In the gastric cancer group, the probability of the test positive was 34.1%. The false negative rate was 65.9%. Due to lack of prevalence of this area, the probability of both positive and the probability of both negative could not be calculated.
