| Objectives Analysis the clinical characteristics and laws of Huaibei Mining Group CWP,Forecast the Mining Group coal workers pneumoconiosis trend, Analysis the mining group losses caused by coal workers ’pneumoconiosis, Analysis the prevention of coal workers’ pneumoconiosis Mining Group tons of coal economic benefits.Methods The object of this research are all of the workers who started exposed to dust from 1970 to 2013 and fit the standard in Huaibei Coal Mine Group. Investigate the General Workers Pneumoconiosis patient-related information, as well as mine dust concentration corresponding year. We established two cohorts by first exposure time(1970-, 1980-), and four cohorts by occupational category(tunneling, mining, combining,and helping), three cohorts by cumulative dust exposure(<100mg · years 、 100-mg · years 、 2000-mg·years). The cumulative incidence of pneumoconiosis was calculated in the same observation period for each cohort of workers exposed to dust by using life table method. Peto Log rank Test was adopted to analyze difference of the incidence between each cohort. We calculated the dose response relationship between CWP cumulative incidence and cumulative dust exposure by using the cumulative dust exposure as abscissa and the cumulative incidence rate as ordinate. The cumulative amount of dust do logarithm transformation, the cumulative incidence density do logit transformed, both seeking linear regression equation. According to the equation anti push-back incidence density estimates in each group segment were forecast under the current dust concentration and incidence of inner life expectancy after the dust, that is the difference between the two is possible to reduce the number of cases. Disability adjusted life years(DALY) include years lived with disability(YLD) and years of life lost(YLL).DALY was used to evaluate life lost caused by pneumoconiosis. Based on analysis of the economic losses caused by pneumoconiosis caused by the "Work Injury Insurance Regulations". In the current coal workers pneumoconiosis case basis, to predict the number of coal workers’ pneumoconiosis may occur in the future, while in 2013 the price of a standard to calculate the economic loss therefore may cause. Dust funding poor economic and irreparable economic loss is obtained. At the same time, reduce the loss of life of healthy persons dust is obtained that is social.Results 1 There were a total of 51538 people in Huaibei Coal Mine Group from 1970 to 2013 who started to exposed to dust. Among them 49421 people data to meet the requirements(CWP 1262 people, non-patient workers exposed to dust 48159 people), the coincidence rate was 95.81%. By the end of 2013, a total of 1262 persons suffered from CWP. In the same observation period of 33 years, 1970- year cumulative incidence queue CWP was 21.4%, 1980-’s cohort was6.5%. The cumulative incidence of various types of workers as are follows: tunneling workers 51.1%, mining workers 28.5%, combining workers 26.8%, and helping workers 9.5%. The cumulative incidence rate for different group of cumulative dust exposure as follows, 2000-mg ? years22.2%, 100- mg ? years8.7%, <100 mg ? years 4.1%. There was a dose-response relationship between cumulative incidence and cumulative dust exposure. Cumulative incidence rate increased gradually with the increase of cumulative dust exposure.2 Dust levels in accordance with 2013, there may be 1358.5 coal workers’ pneumoconiosis within the life expectancy(tunneling workers 431.6 people, mining workers 429.2 combining workers 360.8, and helping workers 136.9). We calculated thecumulative dust exposure for every tunneling worker until their retirement assuming that dust concentration at the 20% of 2013 level, it’s likely there are human cases679.3(tunneling workers 204.4, mining workers 200.2, combining workers 294.9, and helping workers 113.2), reducing the total incidence of 679.2 people.3 From above we concluded that total economic loss was 2,581,838 892.69 yuan caused by 1262 CWP patients who started to expose to dust from 1970 to 2013, of which direct economic losses accounting for 79.74 percent. The total DALY of 1262 cases of pneumoconiosis patients was 9189.03 years, which average healthy life lost were 7.28 years, which lead to disability due to 8484.69 years YLD, and 704. 34 years YLL.4 Through the survey, the first year of an average total cost of ore dust need to invest 5.4million yuan, a year later into equipment maintenance and other expenses of $ 2.6 million will be able to make current workplace dust concentrations decreased by 80%. Currently the coal miners exposed to dust until retirement, the average time for the dust 16.30 years,funds need to be invested 683 million yuan. The investment enables the reduction of 680 CWP, and can save 1.531 billion yuan of economic losses and 848 million yuan to obtain economic benefits.Conclusions 1 As time of first exposure to dust went by, coal workers’ pneumoconiosis incidence rate gradually decreased in Huaibei Coal Mine Group. With the increase of cumulative dust exposure, the cumulative incidence rate increased gradually. There was a dose-response relationship.2 There would be 1359 new CWP patients among the coal workers who started to expose to dust from 1970 to 2013, if the dust concentration kept at the level of 2013. If advanced measures of dust control had been taken, the dust concentration would decrease eighty percent of the present level. 680 coal workers would be free from CWP.3 The average economic loss for each patient would be about 2.0458 million yuan at the price of 2013. Among them direct economic loss would be 1889 thousands yuan, and indirect economic loss would be 1.6313 million yuan. 7.28 years of healthy life would be lost for every patient.4 If additional 683 millions yuan for dust protection were invested in the original 16 mine of Huaibei Mining Bureau subordinate, 1.531 billion yuan would be recouped. 848 millions yuan of benefit and 5950.00 years of healthy life would be obtained. |
